We have been working in the South King County area since 2012 and have a strong record of community-led projects and programs.
OVERVIEW OF POSITION
This position will work on the SAFARI (Sustainable Alliances for African Refugee and Immigrants)-Health Program to improve health outcomes and address health disparities by serving community members who have Health Related Social Needs (HRSNs) and care coordination and resource navigation support and referrals. Studies show that 80 percent of health outcomes are driven by factors that occur outside of the healthcare delivery system - the Social Determinants of Health (SDoH). Providing access, services, and support to help people meet their SDoH and Health Related Social needs (HRSNs) in culturally relevant and appropriate ways helps people achieve the health outcomes that they desire for themselves while improving access to, and engagement with, the healthcare system. Provide outreach, peer support, and referrals for African and Middle Eastern individuals and families to be connected to resources available in our region. You will also have the opportunity to support overall Mother Africa ongoing events and activities. SAFARI Case Manager- supporting language community members to meet their health related social needs (HRSN) through intake, case management, advocacy, referral, and connection to existing resources for employment, transportation, housing, health, education, childcare, etc. Outreach to members of the community who may have health related social needs (HRSN) Work with and accept referrals from partners in the hub in a timely and effective manner Connect clients to resources and information to meet their individual needs and follow up to ensure they got connected to referrals Meet weekly program deliverables goals and provide metrics Enter client-level data into the hub database system Take case notes about the progress and ongoing case management for each client Attend weekly hub partnership meetings to share information and report out on deliverables Conduct job search for clients, support them in applying and preparing for interviews Collaborating internally across programs to ensure clients receive comprehensive support and are seamlessly connected to the appropriate health resources Engage community members to attend Mother Africa events and workshops Collaborating internally across programs to ensure clients receive comprehensive support and are seamlessly connected to the appropriate health resources Participate in all-staff training sessions; attend agency/staff meetings Coordinate connection with other culturally appropriate services and referrals, build relationships with partner agencies to ensure effective referral process Coordinate interpreter supports as needed Support events and activities at our Center for African Women Answer phone calls and screen clients for referral into MA programs and for external referrals Tracking and recording all items distributed to clients, including gift cards, promotional materials, and other items, in a timely and accurate manner. Represent Mother Africa at external events, presenting, exhibiting, networking, and promoting the organization's mission and values. Perform other duties as assigned by supervisors
EXPERIENCE, QUALIFICATIONS, KNOWLEDGE, SKILLS
Proven track record of strong case management skills and experience serving clients and navigating complex social services systems Bilingual in one of the languages listed above and English. Fluent in reading, writing and speaking Proven ability to relate effectively and respectfully with people from diverse backgrounds and to promote a culturally diverse environment Strong problem-solving skills At least 2 years of college or high school diploma with 2 years of case management experience Proficient in Microsoft Office, internet search engines and databases Willingness to comply with established agency performance standards Compliance with HIPPA and confidentiality practices Must have reliable, independent transportation Ability to complete tasks in a timely manner with minimal supervision Creative thinking, very adaptive, and fast learner Ability to thrive in an ever-changing environment, being flexible to changing conditions Ability to work independently and as a team
PREFERRED QUALIFICATIONS
: Experience working with African communities or living and working in Africa. Experience working on nonprofit projects or teams or events.
