  • State General Funds (SGF) in our Case Management department. The CM
  • SGF will manage a caseload of individuals receiving State General Funds for services and maintain documentation needed quarterly and annually. The CM
  • SGF will make collateral contacts with such community resources, such as law enforcement and medical personnel to obtain additional information, coordinate services, or secure their assistance. The CM
  • SGF will ensure the accuracy and timely entry and distribution of required documentation to all necessary team members, including, but not limited to providers, individuals, and family members.
A Case Manager is an empathetic, collaborative, and action-oriented employee with a desire to make an impact in the community and within our teams.
Schedule:
Enjoy a 4-day, 40-hour workweek (Monday-Thursday) with the flexibility to work remotely, in-office, and in the field. This position requires in-person presence to support essential responsibilities such as annual meetings, quarterly mentorings, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas, and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.
