Full Job Description SUMMARY:
Under the direction of the Team Lead / MGR / or Office Mgr. Peer Housing Nav assists members to gain access to necessary housing, medical, nursing, social, restorative and rehabilitative services to reduce homelessness. Pay rate is $________
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Members receives 4 hours a month of housing Case Management. 3 Hours have to be Face to Face and 1 hour can be Phone/Text/Zoom. 2. Provides intake and assessment for Housing Members -evaluating needs and goals through integrated intake assessment (completed within 72 hours of Due date for papers due.) 3. Oversees Care plan - Assists consumers in developing their care plan which includes measurable and time limited goals, objectives and interventions; and a crisis plan; Reviews Care plans at least once every (3) months and makes the necessary changes; 4. Provides, or refers to other professionals and monitors progress for, housing. 5. Promotes crisis prevention and planning, harm reduction, substance reduction, abstinence and recovery skill-building. 6. Teaches Housing Education and management skills; 7. Provides services to support social/interpersonal relationships and leisure-time skill training; 8. To make sure after member is housed they move to a Housed Case Manager. 9. Ensures consumers are covered by insurance and receive entitled benefits. 10. Completes progress notes within one week of services performed using intake software; 11. Upholds all standards for confidentiality of consumer and records; 12. Submits notice of discharge to Office and completes discharge summary within one (1) week of discharge; 13. Checks e-mail daily and reviews any information disseminated by the agency/management team/community. 14. Attend all required meetings, trainings and supervision. 15. Adheres to Agency policies and procedures
