Job Description

Purpose:
To facilitate the Youth Empowerment Services (YES) Waiver participant's achievement of his/her goals of community inclusion and remaining in their homes. Service(s) may be provided in the home and/or community settings. The service(s) provides overall assistance to the family caregiver in the disability-related care of the client, while facilitating the client's independence and integration into the community.
Education Required:
Bachelor's Degree or Master's degree from an accredited college or university in a social services or human services field, or behavioral sciences required.

Pay Range:
$10 paid training and $24 per hour

Additional Requirements:
Applicants must have a flexible schedule and are required to be available during the weekdays (and weekends if needed). The Community Living Supports will work closely with the local mental health authority, assigned therapists, and community members to provide the clients we serve with the best services possible. Community Living Supports will staff their assigned caseload with their supervisor on a weekly basis to ensure the proper protocols and procedures are being followed.
