A Case Manager coordinates delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs.

General Information:
The Foster Care Case Manager is under the direction of the Foster Care Services Director and is responsible for assisting in meeting the goals, objectives, and mandates of the DCS contract and State and Federal guidelines.
Duties/Responsibilities:
Prepare and submit an Individualized Treatment/Service Plan and updates as required by the DCS Contract Provider Manual. Provide all required information to the foster family. Provide all DCS required documentation in TFACTS within the required time frames. Complete monthly summaries per agency guidelines. Prepare the child/youth for placement with a specific foster family to help with adjustment. Provide case management and coordination of services, including support of the DCS Permanency Plan and participation in the Child and Family Team. Assist in preparing the child/youth for return home or for placement in a stable, nurturing, permanent environment. Assist and support the child in receiving outpatient therapy, if indicated, up to twice monthly. Assist and support the child in receiving medication management, if indicated. Coordinate health services, including arranging and accessing community based medically necessary health services through TennCare and private insurance. Provide educational liaison to interact with the child/youth's educational needs and individualized educational plan. The educational liaison may be the foster parent. Provide individualized recreational activities, daily living skills, and independent living skills. Provide services to help the child's permanency family maintain and enhance parental functioning, parental care, and parental ties unless the child/youth's safety would be compromised. Meet with the child/youth's parent(s) without the child/youth at least one time per month. Assess and support the achievement of permanency and other treatment plan goals. Maintain case records in a timely manner and in accordance with all DCS requirements. Evaluate safety, well-being, and permanency. Monitor and document service delivery. Assess and support the achievement of permanency and other treatment plan goals. Provide culturally relevant and informed services to the child, permanency family, and foster family. Travel primarly in and around the Shelby/Tipton Co area. Some travel to Jackson, TN required. Travel dependent on case assignment location and team meeting location. Participate in the Wayne Halfway House, Inc. Performance and Quality Improvement program as requested. Support the Wayne Halfway House, Inc. mission statement and approach to diversity, equity, and inclusion. Perform any other job-related task as assigned by the Program Director.

Education and Experience:
Bachelor's Degree from an accredited institution with a major in social work or a related field. If the degree is in criminal justice, there must be a juvenile justice component. Master's Degree in the social sciences or volunteer, practicum or intern experiences in programs/facilities that work with children and families preferred but not required. At least five years' experience in a juvenile facility is preferred but not required. Must have a current valid license in the state of employment to transport youth.
Physical Requirements:
Prolonged periods of sitting, standing, and driving. In-person interaction required. Must be able to lift up to 15 pounds at times. Must pass a drug test.
