Program Administrator About Human Edvancement Human Edvancement is a nonprofit organization dedicated to empowering transition-age youth and by providing safe housing, comprehensive case management, life skills development, and supportive services that promote self-sufficiency and long-term success. Through our Transitional Housing Placement Program for Non-Minor Dependents (THPP-NMD), we support current and former foster youth ages 18-21 as they transition into adulthood. Our person-centered, trauma-informed approach helps participants build the skills, confidence, and resources needed to achieve stability in housing, education, employment, financial independence, and overall well-being Position Summary We are looking for a part time Case Manager to provide comprehensive case management services to youth18-21 participating in the Transitional Housing Placement Program for Non-Minor Dependents (THPP-NMD). The Case Manager supports youth in achieving self-sufficiency through individualized service planning, life skills development, housing stabilization, education and employment support, and connections to community resources. Essential Duties and Responsibilities

• Provide comprehensive case management services to assigned youth in accordance with

THPP-NMD

regulations and agency policies.

• Monitor participant progress toward established goals through regular meetings, home visits, and ongoing assessments.

• Connect youth with community resources, including healthcare, mental health services, substance use treatment, legal assistance, public benefits, childcare, and supportive services.
• Conduct home visits to monitor participant safety, program compliance, and housing conditions.
• Complete required progress notes and reports.
• Participate in multidisciplinary team meetings, case conferences, and program staff meetings.
• Respond appropriately to participant crises and assist with crisis intervention and safety planning.
• Participate in on-call rotation or after-hours support, if required.
• Promote participant empowerment, self-advocacy, and long-term independence.
• Perform other duties as assigned. Minimum Qualifications
• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
• Minimum of two years of experience working with foster youth, transition-age youth, or vulnerable populations preferred .
• Experience providing case management and crisis intervention.
• Valid California driver's license, reliable transportation, and proof of insurance.
• Ability to pass required background clearances. Knowledge, Skills, and Abilities
• Strong understanding of adolescent and young adult development.
• Knowledge of trauma-informed care, positive youth development, and culturally responsive practices.
• Excellent communication and interpersonal skills.
• Ability to build trusting relationships with transition-age youth.
• Strong organizational and time management skills.
• Ability to maintain professional boundaries while providing supportive services.
• Ability to maintain confidentiality and exercise sound judgment.
• Ability to work independently and collaboratively within a multidisciplinary team. Physical Requirements
• Ability to travel regularly throughout the service area.
• Ability to sit, stand, walk, bend, lift up to 25 pounds, and use standard office equipment.
• Ability to conduct community and home visits.

Work Schedule:

This position allows for a self-managed work schedule with flexibility in determining daily work hours. However, the Case Manager must remain reasonably available during regular business hours to respond to participants, attend scheduled meetings, coordinate services, and communicate with agency staff and external partners.

Work Location:

Hybrid position requiring a combination of remote work, and onsite visits to participants' homes and community locations within the assigned service area.
