Job Description

If so, we have an exciting opportunity for you to work as a Residential Services Case Manager and make a difference in our community and the lives of the individuals we serve. This is a hybrid position where you will split your work time between the office, community, and home. What You Will Be Doing As a Residential Services Case Manager , you will be responsible for administrative case management functions and supporting team operations by maintaining accurate documentation and ensuring all records meet Home and Community Based Services (HCBS) requirements. This position will provide collaborative, strengths based, trauma informed care to adults. You will assist individuals and families by evaluating, providing support and connecting to resources and services in the community in collaboration with community providers and resources in order to assess and/or provide the appropriate level of care. What We Require A Bachelor's Degree in Social Work, Psychology or other related Human Services Field with a minimum of one year experience in behavioral health care services working with adults is required. Experience working with individuals who have lived in a specialized residential setting and familiarity with HCBS rules and regulations preferred. A valid Michigan Driver's License is required. A strong understanding of the needs of persons with mental illness, intellectual/developmental disabilities, and substance use disorders is needed. You will understand how to implement a strong recovery philosophy and be able to collaborate and develop teams.
