As Premier continues to grow, we are seeking an experienced and highly motivated Senior Waiver Case Manager who can independently manage a waiver caseload while contributing to the continued quality and development of our Case Management program. About the Position The Senior Waiver Case Manager provides comprehensive, person-centered case management for individuals enrolled in Minnesota Home and Community-Based Services (HCBS) waiver programs. This position is designed for an experienced Minnesota waiver professional who already understands the fundamentals of waiver case management and can independently coordinate complex services, maintain high-quality documentation, manage required timelines, communicate effectively with interdisciplinary teams, and respond appropriately to changing client needs. The Senior Waiver Case Manager is responsible for the full case management process, including assessment and service planning, service coordination, monitoring, required contacts, documentation, service authorizations, and ongoing collaboration with individuals served, families, guardians, providers, county partners, and other members of the support team. In addition to maintaining an assigned caseload, the Senior Waiver Case Manager will contribute to strong team practices, quality improvement, effective workflows, and the consistent delivery of culturally responsive, person-centered services. We are looking for someone who can operate with a high degree of independence, exercise sound professional judgment, maintain an organized and compliant caseload, and consistently deliver high-quality case management services. Key Responsibilities Person-Centered Case Management Develop strong professional relationships with individuals served and their families, guardians, providers, and other members of their support networks. Coordinate services using person-centered principles that reflect each individual's strengths, needs, preferences, culture, goals, and informed choices. Participate in assessments and develop, implement, monitor, and update individualized service plans in collaboration with individuals served and their interdisciplinary teams. Support individuals in identifying and accessing appropriate services, providers, community resources, natural supports, and other resources necessary to achieve their identified goals. Advocate for individual choice, self-direction, community inclusion, safety, independence, and quality of life. Waiver Service Coordination Coordinate and monitor waiver and non-waiver services for individuals enrolled in programs including CADI, BI, AC, CAC, EW, DD, and Rule 185 , as applicable to assigned caseloads. Maintain ongoing communication and coordination with service providers, county partners, healthcare professionals, families, guardians, and other interdisciplinary team members. Monitor the effectiveness and continued appropriateness of authorized services and respond when an individual's needs, circumstances, risks, preferences, or service arrangements change. Assist individuals and their support teams in navigating provider concerns, service disruptions, appeals, grievances, emergency planning, transitions, and crisis situations. Facilitate effective coordination across multiple service systems when individuals have complex medical, behavioral health, disability-related, housing, financial, or social service needs. Documentation & Compliance Maintain accurate, complete, timely, and professional case documentation consistent with Minnesota Department of Human Services (DHS), lead agency/county, HCBS, contractual, and Premier requirements. Complete required case notes, Coordinated Services and Support Plans (CSSPs), service agreements and authorizations, reassessments, monitoring activities, and other required documentation within applicable timelines. Complete required contacts and in-person visits in accordance with applicable waiver, county, and program requirements. Ensure case records accurately reflect services provided, client needs, significant developments, decisions, follow-up activities, and progress toward identified outcomes. Maintain organized and audit-ready case files and promptly address identified documentation or compliance deficiencies. Service Authorization & Monitoring Coordinate authorized services consistent with each individual's approved service plan, assessed needs, available resources, and applicable waiver requirements. Monitor service delivery to determine whether authorized supports are being provided appropriately and continue to meet the individual's identified needs. Work collaboratively with providers and support teams to address service gaps, provider changes, service interruptions, and other barriers affecting continuity of care. Monitor applicable service agreements and individual budgets and communicate concerns requiring supervisory or lead-agency consultation. Client Advocacy & Problem Solving Serve as a knowledgeable resource for individuals and their support networks as they navigate Minnesota's long-term services and supports system. Assist individuals in understanding available options so they can make informed decisions regarding their services and supports. Recognize situations requiring immediate intervention, mandated reporting, supervisory consultation, or coordination with county or state partners. Respond professionally and appropriately to complex cases, service disagreements, provider concerns, changes in condition, and other circumstances requiring timely case management intervention. Team Collaboration & Peer Support Work collaboratively with the Waiver Case Management Supervisor and other Case Managers to maintain consistent, high-quality case management practices. Participate actively in team meetings, case consultations, training, quality improvement initiatives, and program development activities. Share practical waiver case management knowledge and effective practices with colleagues. Provide appropriate peer support and assist with the orientation of newer Case Managers when requested. Contribute constructive recommendations regarding documentation practices, workflows, client service, and operational improvements based on direct case management experience. Required Qualifications Applicants must meet the following minimum qualifications:
Education:
Bachelor's degree from an accredited college or university in Social Work, Psychology, Sociology, Nursing, Human Services, Behavioral Sciences, or another closely related field that meets applicable case management qualification requirements.
Minnesota Waiver Case Management Experience:
Minimum of one (1) year of direct, verifiable experience providing Minnesota HCBS Waiver Case Management services, including experience with one or more programs such as CADI, BI, AC, CAC, EW, DD, and/or Rule 185.
Person-Centered Practice:
Demonstrated ability to provide case management using person-centered, culturally responsive, strengths-based, and informed-choice principles.
Documentation & Compliance:
Demonstrated ability to maintain accurate, timely, organized, and professional case documentation and consistently meet required deadlines.
Communication:
Strong written and verbal communication skills and the ability to communicate professionally with individuals served, families, guardians, providers, county partners, and interdisciplinary professionals.
Independent Caseload Management:
Demonstrated ability to independently organize and prioritize a caseload, manage competing responsibilities, meet required timelines, and appropriately escalate complex issues when necessary.
Technology:
Ability to effectively use electronic case management and documentation systems, Microsoft 365, and other technology required to perform case management responsibilities.
Mobility & Field Requirements:
Valid driver's license and access to reliable, insured transportation for client visits, meetings, and other work-related travel.
Background Study:
Ability to successfully complete all Minnesota DHS background study and screening requirements applicable to the position. Preferred Qualifications Candidates will be especially competitive if they possess: Two or more years of direct Minnesota HCBS Waiver Case Management experience. Prior experience managing Ramsey County waiver caseloads is strongly preferred. Demonstrated proficiency with SSIS and MnCHOICES. Experience working across multiple Minnesota waiver programs, including CADI, BI, AC, CAC, EW, DD, and/or Rule 185. Strong working knowledge of Minnesota

requirements, person-centered planning, informed choice, service planning, documentation standards, and applicable timelines. Familiarity with the DHS Community-Based Services Manual (CBSM) and other resources governing Minnesota HCBS programs. Familiarity with Medical Assistance (MA), Medicare, and other public benefit programs affecting individuals receiving long-term services and supports. Experience coordinating services for individuals with complex medical, behavioral health, disability-related, housing, or social service needs. Experience working effectively with culturally and linguistically diverse communities. Bilingual or multilingual skills. Experience supporting peer orientation, training, or knowledge sharing within a case management team. Onboarding & Work Arrangement The initial onboarding and orientation period will be conducted primarily in person at Premier's Roseville office to ensure the successful candidate is fully oriented to Premier's systems, policies, documentation standards, workflows, performance expectations, and assigned caseload. The onboarding period may extend for up to 90 days, depending on the employee's experience, demonstrated proficiency, and readiness to independently perform assigned responsibilities. A hybrid work arrangement may be approved earlier for candidates who demonstrate substantial familiarity with Minnesota waiver case management, particularly candidates with prior experience managing Ramsey County waiver caseloads and familiarity with applicable county processes and expectations. Continued hybrid/remote flexibility is dependent upon successful onboarding, satisfactory performance, documentation quality and timeliness, caseload needs, client responsibilities, and operational requirements.
