Position Summary:

We are seeking a compassionate and detail-oriented IDD Targeted Case Manager to support individuals with Intellectual and Developmental Disabilities (IDD). This role focuses on coordinating services, advocating for client needs, and promoting independence and quality of life through person-centered planning.

Key Responsibilities:

Conduct comprehensive assessments to determine client needs, strengths, and goals Develop, implement, and monitor individualized service plans (ISPs) Coordinate and connect clients to community resources, healthcare providers, and support services Maintain regular contact with clients, families, and service providers to ensure continuity of care Advocate for client rights and access to appropriate services Monitor service delivery to ensure compliance with state and federal regulations Maintain accurate and timely documentation in accordance with Kansas Medicaid and agency standards Participate in interdisciplinary team meetings and case reviews Provide crisis intervention and support as needed

Qualifications:

Bachelor's degree in Social Work, Psychology, Human Services, or a related field (preferred), experience in the field or a combination of both.

Person-centered planning and advocacy Critical thinking and problem-solving Time management and multitasking Cultural competency and empathy Compliance with confidentiality (HIPAA) standards At least one (1) year of experience working with individuals with IDD or related populations (preferred) Knowledge of Kansas IDD waiver services and case management processes (preferred) Strong organizational, communication, and interpersonal skills Ability to work independently and manage a caseload effectively Proficiency in electronic health records (EHR) systems and Microsoft Office Valid driver's license and reliable transportation

Work Environment:

This position may involve a combination of office work, remote work, and community-based visits, including client homes and service provider locations.
