Monday-Friday | 8:00 AM - 5:00 PM No weekends No holidays Position Overview We are seeking a Social Work / Human Services Case Manager to support Medicaid Long-Term Care (LTC) and Comprehensive Program enrollees . This is a hybrid role , combining remote documentation with field-based member visits . Candidates must reside in Homestead / Florida City area (33033, 33032) area and be willing to travel. Key Responsibilities Coordinate case management services for Medicaid Long-Term Care members Conduct comprehensive member assessments using care management tools and data review Develop, implement, and monitor individualized care plans Collaborate with interdisciplinary care teams to achieve optimal health outcomes Track progress and update documentation in accordance with program requirements Travel Requirements Up to 75% field-based travel within Miami-Dade County Travel time paid at hourly wage Mileage reimbursed at IRS rate Reliable transportation required Required Qualifications Case management experience required Bachelor's degree in Social Work or a related Human Services field Candidates must reside in Homestead / Florida City area (33033, 33032) Bilingual Spanish/English required Ability to work independently in the field and remotely Authorized to work in the U.S. Preferred Qualifications Experience in Long-Term Care (LTC) or Medicaid programs Strong organizational, time-management, and multitasking skills Excellent verbal and written communication abilities Comfortable working in a fast-paced, changing environment Training 2 weeks of remote training Monday-Friday | 8:00 AM - 5:00 PM Full attendance required Why You'll Like This Role Stable weekday schedule with no weekends or holidays Hybrid flexibility with paid travel time Meaningful work supporting long-term care members Opportunity to grow within care management and social services Interested?
