Job Summary We are seeking a compassionate, organized, and dedicated TCM to join our team. In this role, you will serve as a vital advocate for individuals with I/DD . You will help clients navigate the Kansas Home and Community-Based Services (HCBS) waiver program system , ensuring they gain access to essential medical, social, educational, and residential services to live fulfilling, independent lives.

Key Responsibilities Assessment :

Conduct and participate in comprehensive strengths-based assessments to identify the unique medical, social, and educational needs of individuals on your caseload.

Development of Plan :

Facilitate the development of the Person-Centered Support Plan (PCSP) alongside the client, their family, and their support team. Develop Behavior Support Plans if needed. Monitor and follow up : Perform ongoing regular face-to-face and phone monitoring to evaluate the quality and effectiveness of waiver services.

Referral :

Link individuals with community resources, grant funds, and local agencies.

Advocate :

Empower individuals and their families by navigating barriers, protecting human rights, and securing necessary community resources.

Maintain Compliance :

Care plans and case notes must be kept up to date to maintain compliance with the state.

Collaborate :

Work closely with parents/guardians, local providers, MCOs, and CDDOs. ______________________________________________________________________________________

Qualifications & Requirements Education :

Bachelor's degree in social work, psychology, human services, or a closely related field is highly preferred (or 4 years of experience in human services field) Background preferred : Minimum of 6 months-1 year of professional experience supporting individuals with intellectual or developmental disabilities. Case management experience is required (1 year). Must provide references from past employers.

Travel :

Must have valid driver's license and a reliable car for community-based visits (we do not transport clients).

Clearances :

Must pass comprehensive background checks, Adult and Child Protection Services registry checks, and have CPR/First aid certificate

Pay:

$35.00 - $40.00 per hour
