Please read description thoroughly:

• This position is primarily remote and on the road.
• Case manager to complete home visitations 4 times out the year per client.
• Provide case management services to individuals and families in need
• Conduct assessments to determine client needs and develop appropriate service plans
• Collaborate with other professionals and agencies to coordinate services and resources
• Advocate for clients' rights and access to necessary support systems
• Monitor client progress and make necessary adjustments to service plans
• Provide crisis intervention and support during emergencies or difficult situations
• Complete required state mandated training
• Required to handle minimum caseload of 30

Experience:

• Bachelor's degree in social work or a related field preferred
• Minimum 2 years of Case management experience
• Case Management certification with minimum 2 years of assessment care planning experience.

Skills:

• Excellent communication skills, both verbal and written
• Strong organizational skills with the ability to manage multiple cases simultaneously
• Compassionate and empathetic approach when working with clients in challenging situations
• Ability to build rapport and establish trust with diverse populations
• Proficient in using computer software for documentation and case management purposes
