Job Responsibilities:
Provide direct case management support focused on coordinating client care and reducing barriers to stability Assist clients with access to healthcare services, including primary care referrals, specialist appointments, and care team coordination Accompany clients to medical appointments and advocate during care discussions as needed Support clients in completing medication assistance applications, waivers, and related healthcare forms Assist clients in obtaining required documentation for benefits and services, including state ID, Social Security cards, and birth certificates Coordinate referrals to housing resources and support clients in maintaining housing stability Connect clients to community resources addressing medical, housing, food, employment, and legal needs Monitor client progress toward healthcare access, benefit enrollment, and housing related goals Participate in client care meetings and collaborate with internal teams and external providers Respond to client crises related to health, safety, or housing and connect clients to appropriate interventions Maintain accurate, timely case notes and documentation to support continuity of care and program requirements Other duties as assignedJob Requirements:
Must be able to transport objects weighing up to 25 lbs. Must be flexible and able to work evenings, weekends, and holidays if required Must maintain a valid driver's license and clean driving history Must maintain any training and certification requirements Must support the teachings of the Catholic Church and embody our organization's mission and culture through behavior and daily interactions Regular and predictable on-site attendance is requiredJob Qualifications:
Bachelor's degree in social work, human services, or related field, or equivalent experience Experience in case management, social services, or client support Strong problem-solving skills and ability to respond quickly with a sense of urgency Knowledge of community resources, benefits, housing, and healthcare systems Excellent communication, documentation, and reporting skills Ability to build rapport and work effectively with diverse populations Skilled in crisis response, de-escalation, and client advocacy Organized, detail-oriented, and able to manage multiple priorities Proficient with Microsoft Office and case management systemsPay:
