In this vital role, you will empower individuals experiencing homelessness by providing comprehensive case management, housing assistance, and supportive services to help them secure and maintain stable housing. Your proactive approach will facilitate client success through personalized planning, resource coordination, and ongoing support, ensuring a pathway to independence for high-risk populations. This position offers an exciting opportunity to make a meaningful impact within diverse communities while utilizing your expertise in social services and housing programs. Duties Conduct thorough assessments of clients' needs, strengths, and barriers related to housing stability and develop individualized housing plans. Assist clients in navigating housing assistance programs such as Section 8, LIHTC (Low-Income Housing Tax Credit), and other local resources. Support clients through the entire housing placement process, including landlord negotiations, lease signing, and move-in coordination. Provide crisis intervention and crisis management to address urgent client needs related to mental health, addiction, or other high-risk situations. Maintain detailed case records using property management tools like OneSite and document progress in compliance with HUD (Housing and Urban Development) regulations and Fair Housing laws. Collaborate with landlords, property managers, social service agencies, and community partners to secure sustainable housing solutions. Offer ongoing client communication, motivational interviewing, and support to promote self-sufficiency and long-term stability. Requirements Proven experience working with individuals experiencing homelessness or from underserved communities in a social work or case management capacity. Strong knowledge of landlord-tenant law, Fair Housing regulations, HUD regulations, and housing assistance programs such as Section 8. Excellent negotiation skills for landlord engagement and client advocacy. Ability to perform crisis intervention effectively with high-risk populations including those with mental health conditions or substance use issues. Proficiency in Microsoft Office Suite (Word, Excel) and property management tools like OneSite or similar platforms. Experience working with culturally diverse communities and understanding the unique challenges faced by high-risk populations. Knowledge of addiction counseling techniques and crisis management strategies. Exceptional client communication skills with an empathetic approach tailored to individual needs.
