JOB DESCRIPTION
Case Manager Assistant Primary Function:
To ensure the quality of programs related to the support plan by assisting the Case Manager. 1. Assist the Case Manager as requested. 2. Assist Case Manager with entering programs and creating various reports using the Practical Health system. 3. Monitoring each person's Practical Health data, activities, etc. 4. File monthly reviews and data collection in the person's main record and in the file cabinet, if applicable. 5. Mail or scan monthly reports, notices of upcoming meetings and other documents as applicable to the conservator and in ICF to the therapists. 6. Complete monthly house inspections. 7. Mail the ISP and other documents as applicable to the conservators. 8. Purge people's notebooks and data collection from file cabinet annually and as needed. 9. ICF- Monitor the support plan programs and the support plan program supplies in the home. 10. Waiver
- send out and monitor weekly QC sheets. 11. Waiver
- Monitor and update on-site notebook in Practical Health. 12. Waiver
- Monitor attendance in Practical Health. 13. Waiver
- Assist with assigning weekly TSIs 14.
