Job Description

This Position is temp to perm. Full time. Job Summary The Case Manager plays a vital role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized care plans, and coordinating resources to facilitate access to necessary services. The Case Manager will work collaboratively with clients to promote their well-being and empower them to achieve their goals. Experience Must have Property management or Case management experience Duties Conduct thorough assessments of clients' needs and circumstances to develop tailored care plans. Provide crisis intervention and management support to clients facing immediate challenges. Offer addiction counseling and support for individuals dealing with substance use issues. Collaborate with healthcare providers and community organizations to ensure clients receive appropriate behavioral health services. Utilize motivational interviewing techniques to encourage client engagement and progress. Supervise and mentor junior staff or interns, fostering a supportive learning environment. Address issues related to childcare, child welfare, and Section 8 housing assistance as needed. Assist clients in navigating Medicare and other public assistance programs. Facilitate workforce development initiatives to help clients gain employment skills and opportunities. Maintain accurate documentation of client interactions, progress, and outcomes. Skills Proficiency in crisis intervention techniques and crisis management strategies. Knowledge of addiction counseling principles and practices. Strong understanding of behavioral health issues and social work methodologies. Experience in supervising staff or volunteers effectively. Familiarity with childcare regulations and child welfare policies. Competence in motivational interviewing to enhance client motivation and commitment. Understanding of Section 8 housing programs and related resources. Knowledge of Medicare processes and eligibility requirements. Ability to develop workforce development programs that align with client needs. This position requires a compassionate individual dedicated to improving the lives of those they serve through effective case management practices. Staff will have a Caseload of 30-40 people of property management clients and about 10-15 STRMU applications weekly to process. Process (STRMU) Short Term Rent Mortgage and Utility Assistance Requests Data Entry Recertification of Housing Clients Completion of Documents and Releases Execution of Resident Lease Property management duties of subsidized housing Properties for to include; Locating Affordable Housing Process Monthly Rent Payments Case Management of Clients Some driving will be required.
