Full-time General Description of Duties:
1. Provides linkage to care services for people living with
HIV/AIDS. 2.
Participates in identifying specific areas for outreach activities and provides guidance and instructions to provide community outreach activities related to serving people living with
HIV/AIDS. 3.
Establishes and maintains collaborative relationships with community agencies. 4. Participates in data-driven discussions and strategic planning with community-based organizations (CBOs), Centers of Excellence (COEs), and local health departments (HDs) to improve linkage and re-engagement outcomes. 5. Sustains partnerships with the HIV medical care community to enhance HIV care management for people living with
HIV/AIDS. 6.
Communicates regularly with community agency representatives who provide resources for people living with
HIV/AIDS. 7.
Reviews target population data to determine best approaches to reach target population. 8. Research new strategies for getting people living with
HIV/AIDS
not engaged in care to access services. 9. Maintains a community calendar of activities, including health fairs, neighborhood gatherings, community events, etc. for participation. 10. Designs outreach presentations. 11. Prepares monthly, quarterly, annual progress reports. 12. Responsible for carrying out mandatory assigned tasks related to public health emergency preparedness and response, training and periodic disaster drills. 13. Outreach and care coordination for people newly diagnosed with HIV/AIDS, including referrals to mental health coordinators and ensuring home stability through third-party organizations such as Sacred Heart and Friends for Life.
Experience:
1. Minimum of 1 year of progressively professional experience supporting community health efforts, preferably within a health-related setting. 2. Bachelor's degree from an accredited college or university in public administration, health services administration, health care management, business management or closely related field; OR 3. An equivalent combination of related education and/or experience. 4.
PROOF OF EDUCATION, TRAINING AND/OR EXPERIENCE IS REQUIRED.
