Job Description

Monday - Friday Reports to:Program Director/Assistant Program Director Position Purpose:
To provide case management for clients through the Texas Pregnancy Care Network (TPCN) program.Duties and Responsibilities:
- Develop program curriculum and lesson plans for parent education.
- Record attendance/progress and document all interaction per case notes.
- Organize and maintain a comprehensive list of client referrals and rosters, ensuring accuracy and currency.
- Establish communication channels through phone calls and home visits to ensure consistent client contact and follow-up.
- Facilitate regular communication with clients to provide guidance, support, and encouragement.
- Foster positive relationships with clients from diverse cultural backgrounds.
- Collaborate with program staff, providing activity updates and receiving constructive feedback.
- Attend mandatory agency meetings and training sessions.
- Supports and participates in major fundraising projects and programs.
- Uphold strict confidentiality in line with contractual obligations and agency policies.
- Demonstrate consistent dedication to achieving program outcomes and fulfilling time commitments.
- Keeps current knowledge of counseling methods.
- Keeps current knowledge base as relates to pregnancy and prenatal care, fetal development, parenting, adoption, abortion risks and procedures, risks and complications of other birth control methods, abstinence, sexually transmitted diseases and community resources.
- Communicates well with health care and auxiliary personnel.
- Maintain current Texas Pregnancy Care Network training.
- Provide crisis intervention information, pregnancy support, and referrals for clients according to established protocol.
- Administer urine pregnancy tests following Catholic Charities of the Texas Panhandle's protocol on Pregnancy testing in the Client Service Manual.
- Assist clients with material assistance visits according to protocol.
- Provide follow-up and referrals for clients as needed.
- Maintain client files with proper documentation.
- Participates in chart reviews, special committees, and other quality assurance as needed.
- Keeps client services clean and organized. Each staff member will be responsible for individual and team cleaning duties.
- Reports on unsafe conditions in a timely manner.
- Follows Catholic Charities of the Texas Panhandle's policies and procedures.
- Other duties delegated by his/her supervisor.
Qualifications:
- Proficient in English, encompassing speaking, reading, and writing skills.
- Has a sincere desire to reach out to women and their families in crisis pregnancies, especially the abortion-vulnerable and abortion-minded.
- High School Diploma Required. Bachelor's degree in education or social services or a related field preferred.
- Demonstrate understanding of Joseph's Project processes.
- Competent in using Microsoft Office Suite and other relevant software.
- Exceptional organizational skills, with strong attention to detail and the ability to manage multiple tasks simultaneously.
- Proficient in written communication, analytical thinking, and problem-solving.
- Display a proactive and positive work attitude.
- Exercise discretion, maintain confidentiality, and exhibit professionalism consistently.
- Capable of working both independently and collaboratively as part of a team.
- Preferred experience in Case Management and Grant Management (not mandatory).
- Exhibits fairness, honesty, and open communication with colleagues.
- Effective time management and organizational abilities.
- Ability to travel for program training and between multiple programmatic locations.
- Must be available to work some evenings and weekends for activities, outings, field trips and/or agency events.
