Performs minimally complex social service work within the Veterans Program ensuring clients receive timely access to needed support, crisis intervention and resource navigation. Responsibilities include conducting screenings, assessing needs, authorizing appropriate services, and coordinating evidence-based interventions—such as peer groups, family sessions, crisis prevention support, and psychosocial rehabilitation. The Case Manager develops and monitors Person-Centered Plans, facilitates linkage to community and clinical resources, and evaluates the effectiveness of authorized services. Will require a flexible work schedule in order to meet the program's expectation & Veterans' needs. This may include working after hours & weekends. Works under the supervision of the Program Supervisor with moderate latitude for use of initiative and independent judgment. Examples of work performed Assists in accessing medical, social, educational, and other appropriate services.

Assesses client's needs and authorizes services to meet the identified needs. Coordinates Peer Groups and Family Sessions.

Coordinates the development of the Person-Centered Plan.

Provides Crisis Prevention and Management services when needed.
