This is a professional, part-time position requiring knowledge and experience in client-centered case management within the unhoused population. The ideal candidate must have the ability to deliver human services and work effectively with persons at various stages of the continuum of care of Mother Teresa Shelter (MTS), Mother Teresa Transitional House (MTTH), and Representative Payee program clients (Representative Payee).
Ministerial Character:
The Bishop is the visible principle and foundation of unity in the particular Church (The Diocese) entrusted to him. In a unique and visible way, he makes Christ's mission present and enduring as Shepherd of the Christian Community. In order to fulfill his mission, the Bishop employs suitable, chosen collaborators (clerics, religious, or lay people). He shares with them the apostolic mission and entrusts various responsibilities to them. Each position employed in the Diocese of Corpus Christi helps to extend the ministry of the Bishop in particular ways as outlined in the job duties and responsibilities. Therefore, the employee in this position is closely connected to and assists the Bishop in the performance of his ministry and thereby engages in ministry for the church.
General Responsibilities:
Under the supervision of the Operations Supervisor at Mother Teresa Shelter, Inc., the Unhoused Outreach Case Manager is responsible for providing outreach, awareness, and case management services to individuals who fall within the HUD definition of homelessness. The list below reflects the general details considered necessary to describe the primary functions of the job identified and shall not be construed as a detailed description of all the work requirements that may be inherent in the job. As an integral part of the ministry for the Church, this position will support and uphold the philosophy of Catholic ministry and the mission of the Diocese of Corpus Christi in both professional and personal life Acts as a witness to Gospel values by modeling the teachings of the Catholic Church Continuously grow in the ministry by participating in religious ceremonies, training sessions, and reflection activities as directed Perform literal street outreach and build rapport with individuals who fall within category 1 and category 4 of HUD defined homelessness (including but not limited to: help individuals access resources, transportation, documentation retrieval, provide referrals to community agencies, etc.) Conduct outreach and interviews to deem eligibility for potential residents of MTTH Serve as the MTS representative providing awareness about the unhoused population, stay current with resources, and strengthen communications within the continuum of care network (including but not limited to: attending HIP events, participate in the city counting of the unhoused population, continuum of care street outreach efforts, etc. Maintain a monthly calendar of all community partners/agencies who provide services at Mother Teresa Shelter in collaboration with the MTS Operations Supervisor and increase partnership opportunities in collaboration with the Director of Mission Advancement Ensure proper documentation of the case file in accordance with HMIS system or internal CRMS system of the agency; including maintaining general reporting as instructed by the Director of Mission Advancement Provide leadership and support to volunteer mentors working with un-housed clients Participate in inter-agency case management collaborations to share information, resources and problem-solve client challenges and barriers Address client emergency and crises as needed Perform other duties assigned by the
Executive Director Mental/Physical Demands:
Ability to carry, pull, push, lift, or move objects 10 pounds or more Ability to maintain composure under duress; maintain emotional control under stress Must maintain confidentiality of the department
Minimum Qualifications:
Associate degree or related field experience with individuals experiencing homelessness, and/or mental health needs, substance abuse/dependency needs or co-occurring disorders Knowledge of local community resources Strong computer skills; including Microsoft Office, client record management systems Effective organizational and time management skills Valid Texas State Driver's License, vehicle insurance coverage and a good driving record Be a strong communicator, both verbal and written Be able to understand and respect cultural differences Ability to work independently and be self-motivated (often in the field)
Additional Requirements:
Must successfully complete criminal history background check, motor vehicle record review, and drug screening Certification in Safe Environment training provided by the Office for Safe Environment and Child & Family Resources is require Complete a CPR/First Aid certification class.
