Job Description
Performs minimally complex social service work. Reviews Texas Medicaid & Healthcare Partnership (TMHP) internet portal for new and pending assessments. Responsible for conducting screening and evaluation of person(s) seeking admission into a Medicaid-certified nursing facility. Conducts face-to-face evaluation of said individuals to assess need for specialized services, alternate placement options for the individual, and the need for care in a nursing facility or other appropriate setting to address needs or identified disabilities. Works under moderate supervision with latitude for the use of initiative and independent judgment. Daily review of TMHP internet portalContacts Nursing Facilities to schedule assessment appointmentsAssesses client's needs and authorizes services to meet the identified needsCoordinates service deliveryConduct client intake
