Job Description

Job Overview We are seeking a dedicated and energetic Federal Case Manager to join our dynamic team. In this vital role, you will coordinate and oversee case management services for individuals within federal programs, ensuring they receive comprehensive support tailored to their unique needs. Your proactive approach will empower clients to navigate complex social systems, access essential resources, and achieve their personal goals. This position offers an exciting opportunity to make a meaningful impact on high-risk populations, including underserved communities and individuals with disabilities, through compassionate service and strategic intervention. Responsibilities Develop individualized case plans by assessing clients' needs related to social services, behavioral health, addiction counseling, and workforce development. Provide ongoing client communication, offering motivational interviewing and crisis management to support high-risk populations effectively. Coordinate with federal agencies, community organizations, and service providers to facilitate access to resources such as liability insurance, social services, and addiction treatment programs. Maintain accurate and detailed data entry records using Microsoft Office tools and case management software while ensuring compliance with confidentiality standards. Conduct regular follow-ups with clients to monitor progress, adjust plans as needed, and promote positive outcomes in areas such as employment, housing, mental health, and disability support. Deliver culturally competent services by working with diverse communities and individuals from underserved backgrounds. Prepare clear documentation including reports, client correspondence, and progress notes that reflect effective communication skills and attention to detail. Skills Strong knowledge of social work principles, behavioral health practices, and addiction counseling techniques. Proven experience working with individuals from underserved communities and high-risk populations. Excellent client communication skills coupled with motivational interviewing expertise to foster trust and engagement. Proficiency in Microsoft Office applications (Word, Excel, Outlook) along with familiarity with case management systems. Ability to handle crisis situations efficiently through crisis management skills while maintaining professionalism under pressure. Experience working with individuals with disabilities or behavioral health challenges is highly desirable. Demonstrated capacity for data entry accuracy and maintaining detailed records in compliance with confidentiality standards. Working knowledge of Section 8 housing programs, workforce development initiatives, social services networks, and liability insurance procedures. Join us in making a difference!
