Job Description

Case Manager - Residential Youth Treatment Center Are you passionate about supporting children and adolescents through meaningful casework and clinical coordination? YHA is seeking a full-time Case Manager to join our team and provide essential support to youth placed in our residential treatment program. In this role, you'll work directly under the supervision of the Program Director, ensuring high-quality care through coordinated services, accurate documentation, and advocacy for our clients' well-being.
Key Responsibilities:
Coordinate intake, staffing, and discharge processes Monitor and schedule required annual medical, dental, and vision exams Communicate with medical designees, manage all appointment paperwork, and notify legal guardians or conservators Transport residents to necessary appointments and maintain accurate documentation Schedule psychological and psychiatric evaluations Facilitate treatment team meetings and notify appropriate parties Coordinate with caseworkers, probation officers, and schools regarding hearings, education, and care plans Maintain up-to-date and well-organized client records (intake forms, medical records, reports, incident documentation, etc.) Facilitate and oversee delivery of Transitional Living Services for those youth on caseload 14 years and older. Track medication and obtain psychotropic meds as directed by psychiatrists Act as liaison with Desoto ISD, enroll new students, attend ARD meetings Support administrative tasks and ensure general office organization and cleanliness Serve as the designated driver for GHA as needed
What We're Looking For:
Bachelor's degree in Social Work (or related field) Minimum 1 year of experience in casework or youth services (2+ years preferred) Exceptional organizational and communication skills Proficiency in Microsoft Word, Excel, and general computer usage CPR and First Aid certified (or willing to become certified) Experience with trauma-informed care and working with emotionally disturbed youth
Work Environment & Conditions:
Mixed office and field work with flexible hours as needed Must be comfortable interacting with youth in a variety of settings, including during crisis
Pay:
