Job Description

Position Summary:
The HCS/TxHmL Case Manager coordinates, tracks, and documents the delivery of program services for individuals with intellectual and developmental disabilities (IDD) enrolled in the Home and Community-based Services (HCS) and Texas Home Living (TxHmL) waivers. The Case Manager serves as the primary provider liaison between the individual, their legally authorized representative (LAR), the Local Intellectual and Developmental Disability Authority (LIDDA) Service Coordinator, and Texas Health and Human Services Commission (HHSC) auditors. This position offers an entry-level starting opportunity where thorough initial and ongoing training on program regulations will be fully provided. The primary focus of this role is maintaining 100% regulatory compliance, preventing billing utilization errors, and ensuring high-quality, person-centered service implementation.
Essential Job Functions & Duties:
Implementation Plan (IP) & Service Development (30%): Develop, review, and update Provider Implementation Plans (IPs) within required state timelines following the receipt of the LIDDA Person-Directed Plan (PDP). Ensure all IPs contain measurable, person-centered objectives, specific strategies, clear frequencies, and accurate durations matching authorized service codes. Document and calculate natural supports and justify requested paid units to assist in the Individual Plan of Care (IPC) renewal or revision processes. Utilization Review, Billing & Compliance Tracking (25%): Monitor authorized service units on each client's Individual Plan of Care (IPC) weekly to prevent over-utilization or unbillable service delivery. Audit service delivery logs, habilitation notes, and foster/host home documentation to ensure they match
HHSC EVV
(Electronic Visit Verification) or paper billing requirements. Identify documentation deficiencies or gaps in service delivery and implement immediate programmatic corrections to eliminate agency financial payback risks during state audits. Service Monitoring & Client Advocacy (25%): Conduct and document mandatory monthly or quarterly face-to-face service monitoring visits with the individual at their residential site (Host Home, Companion Care, or Own/Family Home). Assess the individual's satisfaction with current services, verify their health and safety environment, and observe provider interaction quality. Attend all scheduled IPC reviews, PDP planning meetings, and interdisciplinary team (IDT) meetings facilitated by the LIDDA Service Coordinator. Personnel Training & Provider Supervision (20%): Train Host Home providers, Companion Care providers, and individualized skills and socialization staff on individual-specific Implementation Plans, behavior plans, and medical protocols. Verify that service providers are executing goals as prescribed and accurately documenting progress or refusals at the time of service delivery. Identify, report, and document allegations of Abuse, Neglect, and Exploitation (ANE) or critical incidents to DFPS/HHSC and agency leadership within required 1-hour timelines. Qualifications & Educational Background (Per Texas Administrative Code):
Education & Experience Criteria:
Must meet one of the following criteria under Texas regulatory standards: A High School Diploma or GED certificate, plus two (2) years of full-time, supervised paid work experience in a programmatic position providing direct services to individuals with intellectual and developmental disabilities (IDD).
Transportation Requirement:
Must have reliable transportation, a valid Texas Driver's License (TDL), and maintain current automobile liability insurance for local field work.
Preferred Asset:
Bilingual skills (English/Spanish) are highly valued but not required.
Technical Skills:
Proficiency using professional software platforms, specifically Microsoft 365 (Word, Excel, Outlook, Teams Calendar), Dropbox, and Adobe applications, with excellent administrative writing, organization, scheduling, and digital charting capabilities. Texas State & W-2
Employment Compliance Requirements:
Must hold a valid Texas Identification Card or Texas Driver's License. Must pass all mandatory state criminal history clearances and registry checks (EMR, NAR, DPS fingerprinting, OIG exclusions list) required for serving individuals with IDD under the Texas Health and Safety Code. Must complete federal employment verification forms, including IRS Form W-4 and Form I-9 (Employment Eligibility Verification). Must comply with company policies, training requirements, and standard employee guidelines.
Physical Demands & Work Environment:
Requires local travel up to 50% of the time to conduct home visits and attend community meetings.
