Education and Experience:
Must have one of the following: Bachelor's degree in social, behavior or human services field from an accredited university; or Associate degree in social, behavior, human service or health-related field; or High school diploma/GED with 2 years of paid or unpaid experience with individuals with Intellectual Developmental Disabilities Must be able to qualify as a Qualified Intellectual Disabilities Professional (QIDP) Preferred to have at least one (1) year of experience in working with individuals with Intellectual and/or Developmental Disabilities Must have valid TX driver's license and be insurable
Job Purpose:
Performs Service Coordination to individuals such as preventing, and managing crisis situations Performs assessments, monitors, assessing needs, service planning and coordinating and making appropriate referrals Monitor and evaluate the effectiveness of care plans and make adjustments as necessary. Maintain accurate and up-to-date client records and documentation.
Job Function:
Maintains regular contact with individuals served to monitor services and identify needs Will identify gaps in services so individual served experiences a continuum of care to the greatest extent possible Develops Initial and Annual Person Directed Plan and makes revision based on the needs of the individual This Organizations Participates with E-Verify
