GENERAL DESCRIPTION
Enhanced Community Coordination provides intensive and flexible support to achieve success in a community setting. Provides pre- and post-transition services. Monitors the person for one year after transition or diversion. Maintains a case load of no more than 30 persons. Enhances the person's natural supports and promotes successful community living. Addresses a person's special needs. Coordinates transportation to and from trial visits with community providers. Conducts HCS service planning team (SPT) meetings at least quarterly, or more frequently if there is a change in the person's needs. Conducts monthly face-to-face visits with the person and monitors the delivery of all services and supports. Conducts at least three onsite visits of community services delivery. Ensures the person receives timely assessments of behavioral, medical, nursing, specialized therapies and nutritional management needs. Monitors all services identified. Monitors critical incidents involving the person and convening the HCS SPT to provide needed prevention or intervention services for the person. Provides service coordination functions as outlined by Texas Administrative Code Title 40, Chapter 2,
Subchapter L:
Service Coordination for individuals with intellectual developmental disabilities (IDD). Duties outlined under this Code include: coordinating the development of each assigned consumer's Person Directed Plan, coordinating the monitoring services, monthly contact with each assigned consumer, and working closely with private and public providers of Home & Community-Based Services.
Additional duties include:
building relationships and partnerships with consumers and family members; assisting consumers in determining outcomes; incorporating the values of Person Directed Planning; referring to community resources; maintaining Medicaid eligibility; monitoring services and supports; resolving problems or concerns; and working with timeframes.
EDUCATION/EXPERIENCE
Bachelor or advanced Degree from an accredited college or university with a major in social, behavioral, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. Must have at least two year of experience in providing service coordination to individuals with IDD, including those who have complex medical needs.
EXPERIENCE AND SKILLS REQUIRED
Demonstrated, either through experience or education, ability to work in a rapid pace environment in an organized and professional manner. Has experience in coordinating and/or providing services to individuals with IDD, including those with complex medical needs, in the community. Ability to self-monitor production and quality with minimal assistance. Basic knowledge and familiarity with data systems including business software such as Microsoft Word and Excel. Ability to develop productive relationships with individuals and family members that focus on individual specific outcomes. Ability to recognize and coordinate issues between service systems, and to advocate effectively for individuals served. Organizational and typing skills are a must. The ability to communicate both verbally and in writing. The ability to work as a team member in a diverse environment interfacing with both internal and external customers.
LICENSE/CERTIFICATION
Valid Texas driver's license. Must be insurable on the Center's insurance. No more than 3 moving violations in the last 3 years.
SHIFT:
Monday through Friday 8 a.m. - 5 p.m.
