What You'll Do as a Case Manager Complete client assessments to identify needs and strengths Develop individualized care plans and update them as clients progress Coordinate services, resources, and community support Collaborate with clinicians, treatment teams, and other professionals Monitor client progress and maintain accurate documentation Help client's problem-solve barriers and build essential life skills Communicate effectively with clients, families, and team members Approach every interaction with empathy, professionalism, and a client-centered mindset Qualifications Applicants must meet ONE of the following:Note:
Applicants under the "working toward SUDC or SSW" pathway must obtain their SUDC or SSW license within 24 months of hire. Bachelor's degree with Social Service Worker (SSW) license or Substance Use Disorder Counselor (SUDC/LASUDC) Bachelor's degree in any field with willingness to pursue SSW orSUDC/LASUDC
