We provide a safe, caring, and healing environment for women in recovery from trauma, addictions, challenged family systems, and mood disorders. Housing, employment, education, healthy living and reunification are the core pillars upon which our services are based. Through intensive case management we are able to render support for up to 30 women at a time in our 6-month Residential Program. Program participants then graduate to our Aftercare Program, which offers stepped down case management support. Our overarching goals for the women in our program are to illuminate a path for them to successfully re-enter the community, reunite with their families, and address barriers to living more fulfilling and rewarding lives. Overall, women who re-enter the community without support have a recidivism rate of 70%. However, women who complete the Residential to Aftercare Program continuum at Friends of Guest House have a recidivism rate of approximately 15%. Case Manager The Case Manager works as part of the client management team to facilitate all client engagement and case file maintenance, along with individualized treatment planning and implementation.
Position Responsibilities:
Case management Conduct program orientation for new residents within the first three days of arrival Conduct intake meetings with assigned residents and develop an Individual Service and Goal Plan for each new resident within 7 business days of their arrival Provide one-on-one case management services to residents, including advocacy, information and referral services (e.g., employment, housing, social services) (minimum case load of 10 residential clients) Aide clients in establishing daily schedules and routines; help client develop other independent life skills Facilitate the development of financial literacy and budgeting skills with the residents. Maintain bi-weekly case notes and weekly progress notes as mandated by Guest House and the Virginia Department of Corrections (DOC) policies Maintain case files and other required documentation according to agency and DOC policies Regularly identify and maintain relationships with new employment, housing, medical, and other community resources Communicate regularly with relevant stakeholders as part of a resident's treatment team (e.g., Probation Officers, Resident Specialists, etc.) Facilitate psycho-social groups as part of the Workforce and Life Development program schedule Other duties as assigned Potential supervisory functions Serve as administrator for the client database system Regularly run reports to ensure the timely completion of required documentation (case notes, service tracking, etc.) Assist with grant writing and reporting as required by Director of Marketing and Development Maintain the safety regulations of facilities as mandated by DOC and Guest House Collaborate with the Intake Coordinator to approve or deny former applicants requesting to return to the residential program; advise on other complex application issues Facilitate the internship program, training and onboarding for all Residential and Aftercare interns Determine caseload assignments based on client needs and Case Managers' expertise Provide feedback and input on disciplinary actions for residents, as needed Assist in organizing and scheduling trainings for case management staff Assist in Residential program development and restructuring General Enforce compliance with Guest House rules: drug and alcohol policies, emergency procedures, etc. Assist with fundraising events. Maintain confidentiality at all times. Attend and participate in staff meetings and in-service training. Inform supervisor of client and facility needs and concerns. Work closely with management to avert or resolve problems related to both the clients and the facility. Licenses & Certifications Valid driver's license Certified Substance Abuse Counselor Assistant (CSAC-A) (can be obtained on the job) CPR certification (can be obtained on the job) First Aid certification (can be obtained on the job) Qualifications Bachelor's degree (master's preferred) in social work, social services, criminal justice, or related field. Individuals with Virginia licensure (LPC, LCSW or LSATP) strongly desired, but not required. Five years of professional experience in a human services setting assisting at-risk populations with histories of addiction, mental illness, homelessness, domestic violence, severe trauma, etc.) is preferred. Excellent communication skills, including interpersonal, written, presentation and conflict management skills Excellent organizational and time management skills Abilities to work effectively with people of diverse backgrounds and exhibit a high level of cultural competence Proficiency at Microsoft office Reporting Relationships The Case Managers report to the Director of Case Management.
