About The Lamb Center The Lamb Center (TLC) is a faith based human services organization, dedicated to serving individuals experiencing poverty and homelessness in Fairfax, VA. TLC operates a holistic drop-in center that provides meals, laundry, Bible study, case management and other critical services to its guests. In partnership with Wesley Housing, TLC is also an owner-operator of Beacon Landing, a new permanent supportive housing (PSH) community, currently under construction, estimated to open by the end of 2026. About the Beacon Landing PSH Services Program Beacon Landing's Permanent Supportive Housing (PSH) Services Program is designed to promote long-term housing stability, resident wellness, community integration, and self-directed growth for individuals and households residing at Beacon Landing. The program serves residents who may have experienced chronic homelessness, prolonged housing instability, disabling conditions, behavioral health challenges, trauma, social isolation, or other significant barriers to maintaining housing. The Lamb Center recognizes that stable housing provides a foundation from which individuals can more effectively pursue healthcare, recovery, income stability, employment, personal goals, and meaningful community connections. Consistent with this philosophy, Beacon Landing's supportive services are designed to help residents maintain permanent housing while enhancing overall quality of life through individualized, flexible, and resident-centered support. About the Permanent Supportive Housing Case Manager The Permanent Supportive Housing (PSH) Case Manager provides intensive housing stabilization and supportive services to residents at Beacon Landing Permanent Supportive Housing. Operating within a Housing First framework, the Case Manager supports residents that are coming out of chronic homelessness, experiencing behavioral health challenges, medical vulnerabilities, substance use disorders, trauma, and other barriers to long-term stability. The PSH Case Manager works closely with residents to promote housing retention, wellness, community integration, and resident-defined goals through individualized and trauma-informed supportive services. This full-time, exempt position operates under the supervision of the Case Management Supervisor and collaborates closely with Resident Advocates, Property Management, healthcare providers, behavioral health providers, and other community partners. The PSH Case Manager maintains a maximum caseload of approximately 20-25 residents. Permanent Supportive Housing Case Manager Responsibilities Provide direct case management and housing stabilization services to residents within the Beacon Landing PSH program Maintain ongoing engagement and supportive relationships with residents using Housing First, trauma-informed, and harm reduction approaches Conduct assessments, service planning, goal setting, and ongoing reassessment activities Assist residents with benefits access, healthcare coordination, behavioral health linkage, community resources, and independent living support, including accompanying residents to appointments when appropriate and necessary Support residents with housing stabilization, lease compliance support, crisis intervention, and eviction prevention efforts Conduct home visits, wellness checks, and supportive follow-up in accordance with program procedures and established staff safety protocols Monitor for changes in resident functioning, safety concerns, or housing stability risks and respond proactively Participate in lease-up, move-in coordination, and First 90-Day stabilization activities Coordinate with Property Management regarding housing-related concerns while maintaining appropriate role separation Participate in case conferences, supervision meetings, and operational coordination meetings Assist with crisis response, de-escalation, and emergency coordination as needed Collaboration & Documentation Maintain timely, accurate, and professional documentation in HMIS and internal systems Complete assessments, service plans, progress notes, referrals, incident documentation, and required program documentation within established timelines Collaborate with the Case Management Supervisor, Resident Advocates, healthcare providers, behavioral health providers, Coordinated Entry, FCRHA, and community partners and document in HMIS. Maintain appropriate confidentiality and professional communication standards Support operational coordination related to resident engagement, housing stabilization, and continuity of care Other Responsibilities Support a safe, respectful, and resident-centered housing environment Participate in staff training, supervision, quality improvement efforts, and program development activities Assist with operational coverage and crisis response coordination outside of traditional work hours as needed; work alternate scheduled times some early days some late days to meet our clients' needs Perform other duties related to supportive housing operations and resident stabilization as assigned Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Behavioral Health, or related field preferred; equivalent combination of education and relevant professional experience may be considered Minimum of 2 years of experience working in homelessness services, supportive housing, behavioral health, case management, or related human services field preferred Experience working within Housing First, trauma-informed, and harm reduction service models strongly preferred Strong communication, organizational, crisis intervention, and de-escalation skills Ability to work effectively with individuals experiencing chronic homelessness, behavioral health conditions, substance use disorders, trauma, and complex medical needs Experience with HMIS documentation and community-based service coordination preferred Ability to maintain professional boundaries, manage multiple priorities, and work effectively within a team-based supportive housing environment Valid driver's license preferred Work Environment Primarily office/community based Regular interaction with residents, including home visits Exposure to emotionally difficult situations, including interactions with residents experiencing trauma, behavioral health challenges, substance use, grief, or significant housing instability Physical Requirements sitting/standing computer use walking through the property ability to travel to resident homes/community locations ability to respond to emergencies consistent with the role Benefits This full-time position offers generous medical, dental, and retirement benefits Annual wellness allowance Tuition reimbursement Paid leave Supportive work environment
