Pathway Homes Pathway Homes is a nonprofit, community-based organization providing housing and supportive services to adults diagnosed with mental illness and substance use disorders.. We are looking for full-time Housing Stabilization Case Manager Team Lead in Fairfax County.
WHAT WILL YOU BE DOING?
supporting individuals in successfully transitioning from homelessness into Permanent Supported Housing. assisting individuals with in developing skills to live independently, sobriety, psychiatric stability, housing stability and engagement in the community. engages in ongoing clinical assessments, formulates accurate clinical impressions, and adapts services and interventions accordingly. supporting individuals' ability to remain in housing by assisting them with adhering to lease terms, ensuring compliance with fire and safety standards , and developing positive relationships with neighbors, landlords and the community. advocating for, and supporting individuals in self-advocacy related to leasing, fair housing and reasonable accommodations. coordination of care for assigned individuals to include assistance with completing referrals to needed services and supporting the individual's access to available community resources. providing direct supervision to case managers Required qualifications : Bachelor's degree in social work, counseling, psychology or related field. Minimum of three years experience working with individuals with co-occurring mental health and substance use disorders, and/or experiencing homelessness. Must be able to pass a criminal background check and driving record check.
Preferred Qualifications :
