About us Virginia Women and Family Support Center is a 501 (c) 3 nonprofit organization located in Gainesville VA. Our mission is to provide long-term housing to pregnant and homeless women and their children. We invest in stabilizing women and their families to break the cycle of homelessness. We serve the hidden homeless population and give a voice to a group of people who, most often, go unseen. Our program equips mothers with the tools, support, and resources they need to become independent and self-sufficient.
Our work environment includes:
Office setting within the Virginia Women and Family Support Center office complex in Gainesville Residential homes in Haymarket and Manassas. This role is not sedentary and requires a willingness to consistently engage with residents and team members Case Management The basic requirements and responsibilities of this role include coordination of residents and their children's medical, nutritional, fitness and prenatal services, work in conjunction with the administrative staff to organize processes to help residents focus on their goals, medical, social, vocational, educational and therapeutic needs, and adherence to the Covenants at all times when assisting residents. Case Managers will prioritize residents goals, plans and programs at all times. The Case Manager is also responsible for attending staff and Residential meetings, client reviews, supervision, etc. Case Manager works one on one with residents daily: Case Manager will conduct a Career and Personality Assessments to determine residents skills, strengths Conduct Career Assessments Review Career Assessments with Resident(s) Determine what residents strengths and weaknesses are and create educational and career goals around the assessment results and resident career interests. Identify long and short term educational goals Report to Resident Manager Responsibility for keeping the house safe, healthy and well ordered: Knowledge of safety and emergency procedures is a plus Facilitate classes aligned with the program guidelines Participate in daily client meetings and client reviews, weekly staff meetings, additional meetings and other agency related events Proactively triage and assess client needs; Review credit report from all the agencies: Experian, Equifax, and Transunion Make sure all accounts are accurate and they belong to the resident Assist residents with disputing any and all accounts that don't belong to them. Have sample of Credit Bureau dispute letters Handy Once all accounts have been reviewed, begin to discuss creating a budget with the resident Create a Budget. Ask Residents to provide statements on all income earned. (Share with residents the goal is to get them to pay off all their debts while they are in the program where they do not have to pay rent.) Ask Resident to also have a copy of bank statements Have residents list all their debts on the debt sheet. (This will help them see the big big of their financial history and assist in helping them focus on where they want to go financially.) Explain to the resident, the goal is to zero out their income because they have control over how they spend their money. (review Financial Plan(s) Coordinate client care with other providers Develop individual plans to include short-term and long-term goals and strategies Consult with other treatment providers Evaluate client progress, achievement of goals, and discharge planning needs. Monitor and evaluate case load to ensure quality delivery of services to clients. Perform other duties related to this position, as directed by the RM. The main responsibility of a Case Manager at Virginia Women and Family Support Center is ensuring the residents are following our program. Daily interactions with clients will include some of the following tasks: coordination of the clients medical services to include physical and mental health creating goals focused on clients' medical, social, vocational, educational and therapeutic needs assisting in acquiring social services building community outside of the residence providing resources and teaching residents how to find resources keeping accurate records of client progress and interactions updating resident schedules performing checks on order and cleanliness of the residence Additional duties may include conducting intake interviews, answering phones and responding to various inquiries, outreach to pregnancy centers, social workers, and homeless shelters Everyday is different for Case Managers and we encourage our team to take a proactive approach to working with residents, rather than a reactive response to what is happening at the moment. An essential function of this role is the ability to hold others accountable and a willingness to facilitate difficult conversations. This role is part-time and requires availability in the evening and on the weekends.
