Case Manager - Newport News, VA Summary Pyramid Healthcare at Newport News is seeking a dedicated Case Manager to support adults with Substance Use Disorders. This role involves coordinating aftercare appointments, communicating with referral sources, legal entities, and other agencies, and helping ensure continuity of care throughout the treatment process. Responsibilities Assess client needs and arrange appropriate services across the full continuum of care Coordinate aftercare plans, appointments, schedules, and meetings Facilitate halfway house interviews and placements as needed Collect necessary information and releases from clients, families, and referral sources Maintain accurate communication and documentation with external stakeholders regarding client treatment plans and progress Perform additional duties as assigned Required Skills & Experience Bachelor's degree in a related field required; Master's degree preferred Two years of previous case management experience preferred Schedule Monday-Friday with rotating Saturdays 8:00 AM-4:30 PM Compensation $18.00-$20.00 per hour, based on experience
