Full Job Description Position Summary:
The Emergency Shetler Case Manager provides comprehensive, trauma-informed, and survivor-centered case management services to individuals and families housed in the YWCA emergency shelter program. This position is responsible for supporting survivors in achieving safety, stability, healing, and self-sufficiency through individualized service planning, advocacy, crisis intervention, resource coordination, and ongoing support. The Case Manager works collaboratively with internal programs and community partners to ensure survivors receive holistic services that address safety, housing, legal needs, healthcare, economic stability, emotional well-being, and positive exits from shelter.Emergency Shelter Case Manager Role & Responsibilities:
Case Management and Survivor Support Provide individualized, survivor-centered case management services to survivors of domestic violence, sexual assault, stalking, and related forms of victimization. Conduct comprehensive intake assessments to identify survivor strengths, needs, risks, barriers, and service priorities. Develop and implement individualized service plans that support survivor-defined goals and promote safety, stability, empowerment and a stable exit from shelter. Maintain ongoing contact with survivors to monitor progress, reassess needs, and provide continued support. Assist survivors in identifying and overcoming barriers to safety, housing, employment, education, healthcare, and financial stability. Safety Planning and Crisis Intervention Conduct ongoing safety assessments and individualized safety planning with survivors and their families. Provide trauma-informed crisis intervention and emotional support during times of immediate need. Respond appropriately to crisis situations and coordinate emergency services, shelter placement, or community resources as needed. Maintain awareness of domestic violence and sexual assault dynamics, trauma responses, lethality indicators, and barriers to accessing services. Support survivors in developing strategies to increase personal and family safety. Advocacy and Resource Navigation Connect survivors to community resources including housing assistance, counseling services, healthcare, legal advocacy, victim compensation programs, transportation assistance, employment services, childcare resources, and public benefits. Advocate on behalf of survivors with community agencies, landlords, employers, schools, healthcare providers, and government agencies when appropriate. Assist survivors in navigating systems including healthcare, housing, social services, criminal justice, education, and public assistance programs. Provide information regarding victim rights, available services, and community resources. Maintain current knowledge of local, state, and federal resources available to survivors. Goal Planning and Empowerment Collaborate with survivors to establish measurable, individualized goals that reflect their priorities and desired outcomes for shelter exit. Support survivors in building life skills related to financial literacy, budgeting, employment readiness, self-advocacy, and independent living. Encourage survivor empowerment, self-determination, and informed decision-making. Conduct regular case reviews and adjust service plans based on changing circumstances and goals. Celebrate progress and support resilience throughout the survivor's healing journey. Collaboration and Coordinated Services Collaborate with YWCA departments to ensure coordinated and seamless service delivery. Participate in multidisciplinary case reviews, staff meetings, and team consultations. Coordinate services with community partners, including healthcare providers, law enforcement, legal service providers, housing agencies, educational institutions, and mental health professionals. Facilitate referrals and follow-up to ensure survivors successfully access the services needed. Maintain effective partnerships that support comprehensive survivor care. Data, Documentation, and Compliance Maintain accurate, timely, and confidential case documentation in accordance with agency policies, funding requirements, and best practices. Enter required data into agency databases and case management systems. Ensure compliance with confidentiality laws, victim service standards, grant requirements, and ethical guidelines. Track service outcomes, referrals, and program performance indicators. Support reporting, audits, and program evaluation activities as needed. Program Support and Professional Responsibilities Participate in ongoing training and professional development related to domestic violence, sexual assault, trauma-informed care, crisis intervention, and case management best practices. Contribute to continuous quality improvement efforts within the Crisis Services Department. Maintain professional boundaries, ethical standards, and survivor-centered practices always. Support program operations, special projects, and agency initiatives as assigned. Supervisory Responsibilities This position does not have formal supervisory responsibilities but may provide guidance or mentorship to interns, volunteers, or new staff as assigned. Qualifications Education Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Public Health, or a related field preferred. Experience Minimum of two (2) years of experience in case management, victim services, social services, behavioral health, crisis intervention, or a related human services field. Experience working with survivors of domestic violence, sexual assault, stalking, human trafficking, or other forms of interpersonal violence strongly preferred. Experience providing trauma-informed services to vulnerable or marginalized populations required. Experience coordinating community resources and support services preferred. Knowledge, Skills, and Abilities Strong commitment to trauma-informed, survivor-centered, and culturally responsive practices. Knowledge of domestic violence, sexual assault, stalking, trauma, and victimization dynamics. Knowledge of crisis intervention, safety planning, and risk assessment principles. Strong case management, advocacy, and resource coordination skills. Ability to establish rapport and build trusting relationships with survivors. Excellent written, verbal, and interpersonal communication skills. Strong organizational, problem-solving, and time management skills. Ability to manage multiple cases and prioritize workload effectively. Proficiency with case management databases, documentation standards, and Microsoft Office applications. Ability to work independently and collaboratively in a multidisciplinary team environment. Ability to maintain confidentiality and exercise sound professional judgment. Additional Requirements Successful completion of criminal background and reference checks. Completion of required domestic violence and sexual assault advocacy training upon hire. Valid driver's license and reliable transportation required for community-based services and client support activities. Ability to work occasional evenings or weekends as needed to meet program and survivor needs.
