Job Description
Job Summary We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing comprehensive support and guidance to individuals and families in need of assistance. This role involves assessing client needs, developing personalized service plans, and coordinating resources to promote stability and well-being. The Case Manager will work closely with clients from diverse backgrounds, ensuring they receive the necessary services to improve their quality of life. Responsibilities Group Facilitation Verification of Benefits Conduct thorough assessments of clients' needs and circumstances. Develop, implement, and monitor individualized service plans. Provide crisis intervention and support during challenging situations. Facilitate access to community resources, including housing assistance, behavioral health services, and workforce development programs. Supervise and support clients in navigating complex systems such as Section 8 housing and child welfare services. Maintain accurate documentation of client interactions, progress, and outcomes. Collaborate with multidisciplinary teams to ensure holistic care for clients. Advocate for clients' rights and needs within the community. Qualifications Experience in social work, behavioral health, or crisis intervention is highly desirable. Strong mentoring skills with a focus on workforce development initiatives. Excellent communication skills, both verbal and written. Ability to work independently as well as part of a team in a fast-paced environment. Proficient in Google Suite Strong organizational skills with attention to detail. Active or Pending LCADC or QMHP-A preferred.
