This dynamic position combines structured case management with personalized life coaching, helping participants battling addiction and co-occurring mental health conditions build the practical skills, resources, and mindset needed for lasting change. You will guide participants through a defined case-planning process — from intake through case closure — coordinating care and connecting participants with resources across every major life domain. We are committed to fostering an environment that promotes growth, compassion, and impactful participant care. Responsibilities Intake, Assessment & Orientation Complete intake screening and orientation with new participants. Conduct assessments to inform individualized case planning. Build rapport with participants during the initial "Get to Know Each Other" phase of engagement. Case Plan Facilitation Develop and guide participants through a structured, individualized case plan covering: mental and physical health and sobriety; ID recovery, benefits, and referrals; finances; employment; housing; education; family reunification; healthy support systems; and faith-based support (as applicable and participant-selected). Meet with each participant for a goal of one hour per week to review progress and move the case plan forward. Facilitate dedicated life coaching modules, including preparing for change and understanding one's past, present, and future, using strengths-based, motivational coaching techniques. Track participant progress through each case plan stage through appropriate documentation and our case notes process. Coordination & Documentation Coordinate with healthcare providers, benefit agencies, community organizations, and family members to support case plan goals and integrated participant care. Maintain accurate documentation in compliance with HIPAA regulations, including progress notes and case plan records, using appropriate ICD coding where applicable. Engage in data collection related to participant outcomes to support continuous quality improvement initiatives. Create and store case notes and required documentation within Progress House systems within 24 hours of meeting with a participant. Recognize signs of crisis or acute mental health/substance use needs and connect participants to appropriate clinical or emergency support, consistent with agency safety protocols. Educate participants on relapse prevention strategies and available community resources to promote sustained recovery.
