Position Summary The Housing & Employment Case Manager works directly with clients to identify opportunities, develop individualized plans, and provide guidance to secure permanent housing and gainful employment . This position combines office-based responsibilities with fieldwork in the Yakima community and requires strong problem-solving skills, empathy, and the ability to inspire hope. Key Responsibilities Communicate effectively with clients, referral sources, and community partners. Maintain current knowledge of FCS housing and employment services and procedures. Work with clients to identify opportunities, develop goals, and implement individualized housing & employment plans. Assist clients with housing applications, supportive/subsidized housing paperwork, and the employment application process. Prepare clients for job interviews and provide training as needed. Maintain accurate client records and case notes in compliance with HIPAA guidelines. Monitor client progress and revise plans as necessary to ensure success. Attend program-related community, coalition, and committee meetings as assigned. Conduct crisis interventions when necessary. Collaborate with other case managers to ensure coordinated and effective service delivery. Continually survey the job market and housing resources to support client goals. Required Qualifications Bachelor's degree in human services or related field, or equivalent combination of education and work experience. Experience in social services or case management, including familiarity with case planning and documentation. Knowledge of local housing and employment resources. Experience working with individuals experiencing homelessness or housing instability preferred. Proficiency in Microsoft Word, Excel, and other standard office applications. Valid driver's license, proof of insurance, and reliable transportation. Physical & Environmental Requirements Ability to remain stationary for portions of the day (>50%) and move throughout office and community locations. Frequent use of computer, office machinery, and communication devices. Ability to observe and assess clients in their living spaces. Occasional lifting of up to 25 lbs. Local travel and outdoor work are required; exposure to weather is expected. Mental & Emotional Requirements Strong time management and organizational skills.
