Full Job Description Job Summary:
The Domestic Violence Rapid Rehousing (RRH) Case Manager facilitates participant enrollment into the domestic violence Rapid Rehousing program and provides comprehensive, trauma-informed case management both in the home and community. This role focuses on developing individualized self-sufficiency plans and coordinating tailored services to ensure long-term housing security for survivors. The Case Manager addresses immediate crisis safety needs while working towards reducing barriers to permanent, stable housing
Hours:
Full-time. Hourly 10:00 am to 6:00pm The ability to work flexible hours is necessary. Primarily works business hours during the week, but duties may require working some evenings and weekends.
Essential Duties:
Manage the program intake process to screen, assess, and officially enroll domestic violence survivors into the Rapid Rehousing program utilizing coordinated entry. Provide ongoing, holistic support that directly addresses the unique emotional, physical, and crisis safety needs of survivors. Provide visits in the homes of survivors and teach the skills needed for independence. Partner with survivors to develop comprehensive, flexible safety plans tailored to their unique workplace, housing, and community settings. Collaborate with survivors to develop actionable goal planning pertaining with finances,employment, education, This includes creating personal goals designed to foster independent living for long term success. Build and sustain a network of local property managers and landlords willing to partner with the Rapid Rehousing program. Coordinate the timely allocation of temporary rental assistance, security deposits and utility payments. Complete and maintain necessary documentation including coordinated entry, osnium, and client case files. Follow all procedural policies and processes as required with this grant. Maintain a clean, professional appearance of facility by following protocol for accepting and storing donations, keep storage areas organized, and complete other tasks as assigned. Maintains the confidential nature of client and business information.
Knowledge, Skills, and Abilities:
Knowledge of the dynamics of domestic violence, poverty and trauma informed care. Knowledge of human development; strong understanding of attitudes, behaviors and development. Understand the individual and systemic needs domestic violence victims experience. Ability to understand human behavior and to relate sensitively and effectively with victims of domestic violence and their families. Knowledge of building community partnerships with employers, educational institutions, landlords, and other community partners. Ability to establish and maintain cooperative working relationships with a wide variety of collateral resources and internal staff. Knowledge of and ability to utilize a computer and the required software. Ability to communicate organization needs in a manner that encourages others to partner and donate. Ability to be decisive and make constructive decisions, under the pressure of crises or emergencies, with appropriate knowledge and timing. Ability to plan and organize work and to make effective use of time. Ability to be cross trained in a variety of positions to assist with all aspects of our work. Ability to communicate effectively both orally and in writing. Ability to establish and maintain positive working relationships with internal staff and the broader community. Ability to work the required hours of the position consistently. This position's primary hours will be from 10am-6pm. Duties require flexibility with hours and some evenings and weekends if necessary.
Minimum Qualifications Required:
Bachelor's Degree in social work or related human service field or equivalent human service related experience. Experience with in-home visitation and independent living skill training preferred Experience in providing direct services to persons of varied backgrounds, ages and ethnicity is required. Strong knowledge of local housing programs and community resources is preferred. Valid Wisconsin driver's license and insurable per Golden House auto insurance requirements Completion of criminal background check.
Typical physical and mental demands:
Requires sitting, standing, bending and reaching. Requires eye-hand coordination and manual dexterity sufficient to operate a computer and standard office equipment. Requires hearing within normal range and vision correctable to 20/20 to read communications, reports and computer terminals. Requires lifting at a minimum of 30lbs. Requires use of cleaning products both in shelter and in community living apartments. Must be able to analyze many variables and choose the most efficient and effective course of action. Must be able to give, receive and analyze information, formulate plans, prepare written materials, and articulate goals and action plans. Must be able to communicate and provide verbal feedback in a positive, professional manner. Must be able to resolve problems, handle conflict, and make effective decisions independently.
