Job Description
Make a Difference in the Lives of Older Adults Are you passionate about helping others navigate resources and access the benefits they deserve? Manitowoc County's ADRC of the Lakeshore is seeking a Part Time Elderly Benefits Specialist to serve in Kewaunee County. This position will work approximately 23 hours/week. Position Summary The Elderly Benefits Specialist serves the older adult population of Kewaunee County by identifying individual needs, advocating on their behalf, and assisting them in securing benefits and services. This position provides information, guidance, advocacy, and support to older adults, their families, and caregivers while empowering them to make informed decisions and access available community resources. Key Responsibilities Provide information, assistance, and advocacy to older adults, their families, and caregivers.
Assess individual needs and identify programs, services, and community resources that can provide support.
Assist clients in understanding and applying for benefits and services for which they may be eligible.
Make referrals to appropriate agencies and community partners.
Empower individuals and caregivers to become effective self-advocates.
Conduct follow-up with clients to ensure referrals and services are meeting identified needs.
Provide ongoing support and additional referrals when necessary to help clients achieve positive outcomes.
