Job Description
Appalachian Community Health Center is recruiting for a full-time Waiver Case Manager to provide services to the Center's ID/D Waiver members. Services will include home visits, team meetings, working with other providers and a significant amount of computer entry/documentation. A person in the position will work as a part of a team in providing these services. This is a new opportunity. The position now requires either a four year degree in a human services field or a high school diploma and 5 years experience with individuals who have intellectual disabilities. It also includes any combination in the above qualifications including 2 year degrees or no degree with a LSW. This will now provide an excellent opportunity for individuals without a formal 4 year human services degree to start providing a higher level of services to the ID/D Waiver member.
