Job Description

Qualifications:

Four year degree in a human service field or four year degree in a non-human service field and one year experience in the I/DD field Possess strong communication skills (written and verbal) Possess basic math skills and strong critical thinking skills Possess basic computer skills Detail oriented, strong organizational skills and excellent time management skills are required A valid driver's license, automobile and insurance The Case Manager will maintain a case list of assigned clients by providing the following: Coordinate and conduct monthly visits as required by funding streams Coordinate and conduct IPPs (annual, 6 months, two 90-days, and other IDTs) Meet Case Management Department billing expectations Comply with daily documentation requirements to include standards of documentation and efficiency in submission Assess client needs and monitor service plans Monitor client services provided and ensure compliance to approved budget and IPP Provide linkage and referral, and keep current with service providers Facilitate communication among team members Maintain client records in accordance with Medicaid and agency regulations Attend Case Management meetings Keep supervisors abreast of client issues Client advocacy Work effectively with teams and supervisors to handle day to day and crisis situations Maintain working knowledge of funding streams, resources, paperwork and diagnoses Travel as required (potentially for training, conferencess, etc.) Any Additional duties as assigned by supervisor
