The Case Manager will maintain a case list of assigned clients by providing the following: Coordinate and conduct IPPs (annual, 6 months, 90-day reviews, and other IDTs) Coordinate and conduct monthly contacts as required by funding streams Comply with documentation requirements to include standards of documentation and filing Monitor client services provided and ensure compliance to approved budget and IPP Provide linkage and referral, and keep current with service providers Facilitate communication among team members Maintain client records in accordance with Medicaid and agency regulations Attend Case Management meetings Keep supervisors abreast of client issues Client advocacy Work effectively with teams and supervisors to handle day to day and crisis situations Maintain working knowledge of funding streams, resources, paperwork and diagnoses. Travel as required (home visits and potentially for training, conferences, etc.) Additional duties as assigned by supervisor
Qualifications:
Preferably a four-year degree in a human service field or four-year degree in a non-human service field and paid or unpaid experience in the I/DD field. Non-degreed applicants may be considered based on work or other experience. Complete a state mandated online training with passing test Possess strong communication skills (written and verbal) Possess basic math skills and strong critical thinking skills Possess basic computer skills Detail oriented, strong organizational skills and excellent time management skills are required An automobile, valid driver's license, insurance, registration and inspection sticker Must pass a drug test and a WV Cares background check
