The Navigator provides trauma-informed, person-centered support services to individuals experiencing homelessness, housing instability, mental health concerns, substance use challenges, and other barriers to stability. The Navigator assists clients with accessing community resources, attending appointments, completing program requirements, increasing independence, and progressing toward individualized goals related to housing, employment, recovery, and overall wellness. This position requires the ability to collaborate effectively with clients residing within emergency shelter, transitional housing, supportive housing, and community-based settings. The navigator will collaborate with therapists, shelter staff, case managers, navigators, peer recovery support staff, medical providers, and community partners to help reduce barriers to treatment and improve client outcomes. This may include going to Huntington City Mission and interacting professionally with clients, residents, guests, and staff as part of service coordination, engagement, outreach, and treatment support. Navigators are expected to uphold the core values of service, dignity and worth of the person, integrity, competence, human relationships, social justice, confidentiality, professionalism, cultural sensitivity, and client self-determination in all interactions and services provided within Huntington City Mission and associated behavioral health settings.
DUTIES / POSITION ACCOUNTABILITIES 1.1
Client Engagement & Support Build professional, supportive relationships with clients using trauma-informed and person-centered approaches. Assists clients in identifying strengths, needs, goals, and barriers to stability. Encourages active participation in services, appointments, treatment planning, and program expectations. Provides emotional support, encouragement, advocacy, and accountability to clients. Always maintain appropriate professional boundaries. 1.2 Care Coordination & Resource Navigation Assist clients with accessing community resources including: Housing Benefits Employment services Medical care Behavioral health services Recovery supports. Transportation Food assistance Legal resources Identification documents Coordinates with internal and external providers to support continuity of care. Help client's complete applications, referrals, releases of information, and required documentation. Assists with appointment scheduling and follow-up services. 1.3 Transportation & Community Outreach Provides transportation assistance for approved appointments and program-related activities as assigned. Assist clients with navigating community systems and attending appointments independently when appropriate. Conducts outreach and engagement activities within the community assigned. Represents the organization professionally during community interactions, meetings, trainings, and events. 1.4 Documentation & Compliance Completes documentation accurately and within required times. Maintains client confidentiality in accordance with HIPAA, organizational policy, and applicable regulations. Documents client interactions, supportive services, transportation activities, referrals, and progress notes as required. Assists with data collection, surveys, program reporting, and outcome tracking when assigned. Maintains compliance with HUD, ESG, CoC, HMIS, Medicaid, SAMHSA, and agency requirements as applicable. 1.5 Team Collaboration Works collaboratively with therapists, navigators, case managers, peer recovery support specialists, shelter staff, and administration. Must be able to work as directed. Participate in staff meetings, case staffing, training, supervision, and program planning activities. Communicates professionally with staff, clients, community partners, and stakeholders. Assists with crisis intervention and de-escalation efforts when needed. 1.6 Professionalism & Ethics Demonstrates professionalism, dependability, and ethical conduct. Maintains good attendance and reports to work on time and ready for duty. Follows agency policies, procedures, and safety expectations. Maintain professional and ethical boundaries in all interactions with clients, Maintains confidentiality regarding patients, employees, and organization. 1.7 Community Engagement & Additional Responsibilities Assists with tabling events, community outreach activities, donation drives, and resource events. Participates in community collaboration efforts and networking opportunities. Provides feedback to leadership regarding program needs, barriers, and opportunities for improvement. Perform additional duties as assigned by the Director of Behavioral Health or administration.
QUALIFICATIONS
High school diploma or GED required. Bachelor's degree or higher in social work, Human Services, Psychology, Counseling, or related field preferred. Experience collaborating with individuals experiencing homelessness, substance use disorders, mental health concerns, or crisis situations preferred. Valid driver's license and insurable driving record preferred/required based on position duties. Ability to work independently and collaboratively within a multidisciplinary team. Effective communication, organization, and people skills.

Knowledge of trauma-informed care and community resources. Ability to engage individuals from diverse backgrounds with dignity and respect. Ability to maintain confidentiality and professional boundaries. Ability to manage crisis situations calmly and appropriately. Ability to complete documentation accurately and timely. Basic computer and technology skills required.
WORK ENVIRONMENT / PHYSICAL REQUIREMENTS
Frequent walking, standing, sitting, bending, and lifting. May require transporting clients and traveling throughout the community. Exposure to behavioral health crises, homelessness, substance use, and high-stress situations. Ability to safely operate agency vehicles if assigned driving duties.
