Job Description

Job Overview Under the direction of the Executive Director, the Community Outreach Coordinator will serve as the primary point of contact for day-to-day program case management and coordination of services that improve mental health status, well-being, financial stability, and social supports that reduce risk of suicide for eligible veterans and active-duty service members in Greene, Hale, and Tuscaloosa county. The coordinator is responsible for program oversight, participant screenings, referral coordination, detailed documentation, and collaboration with the regional veterans affairs(VA) office and community organizations. The coordinator collaborates with leaders and content experts to establish priorities, execute strategies, and implement programs. The individual in this role is expected to identify opportunities to improve outreach activities, strengthen community partnerships, and increase program effectiveness, using critical thinking and problem-solving skills to ensure program success. Summary of Responsibilities; ▪ Assess on an ongoing basis the needs of eligible individuals and their families for suicide prevention services, ▪ Coordinate the provision of suicide prevention services with VA services for which eligible individuals are also eligible, ▪ Tailor (i.e., provide individualized) suicide prevention services to the needs of eligible individuals and their families. ▪ Seek continuously new sources of assistance to ensure the continuity of suicide prevention services for eligible individuals and their families as long as the eligible individuals are determined to be at risk of suicide. Responsibilities will include but are not limited to ; ▪ Conducting outreach and screenings, coordinating with community partners and the VA, supporting participant enrollment activities, maintaining communication with referral partners ▪ Maintaining regular contact with participants to monitor progress, identify new needs, and help ensure veterans remain connected to services and support that promote wellbeing, stability, and reduced suicide risk ▪ Documenting program activities, coordinating educational and outreach events, and assisting with program reporting and implementation activities. ▪ Working directly with participating veterans and family members to assess needs, coordinate referrals, assist with accessing VA healthcare and benefits, connect participants to supportive services and public benefits ▪ Providing follow-up contacts, monitoring participant progress, and supporting ongoing case management activities to help address barriers affecting wellbeing and stability. ▪ Referrals and service coordination through established communication channels, warm hand-offs, follow-up contacts, and regular coordination with participating organizations to help ensure veterans and family members are connected to appropriate services and resources in a timely manner ▪ Conducting ongoing monitoring and periodic site visits to help ensure services are delivered as intended and that vendors and collaborating organizations remain aligned with program requirements and objectives ▪ Utilizing VA-required screening and assessment tools to identify suicide risk factors, behavioral health concerns, barriers to care, and other service needs ▪ Working with participants to develop realistic, achievable goals and identify steps needed to connect them to appropriate services and resources ▪ Connecting veterans to VA healthcare, behavioral health services, benefits, housing resources, employment assistance, transportation support, or other community resources Education Bachelor's degree required in Social Work, Social Services, Public Health, Healthcare Administration, Behavioral Health, Human Services, Business Administration, or a closely related field. Master's degree in a related field preferred. Work Experience Minimum Two (2) years of process improvement and project coordination experience. Experience in case management is strongly desired. Preferred Two (2) years of working veterans, active-duty service members, and/or families. Experience working with Microsoft Excel and Word and G-Suite. Federal and state grant administrative experience preferred. Required Skills, Knowledge, and Abilities ▪ Experience managing multiple projects and collaborating with leaders, community partners, and multidisciplinary teams. ▪ Ability to develop and implement programs and processes that support suicide prevention and improve access to services for eligible Veterans and active-duty service members. ▪ Strong organizational, decision-making, and problem-solving skills, with the ability to manage competing priorities. ▪ Excellent communication, facilitation, conflict resolution, and relationship-building skills. ▪ Ability to work effectively with all populations, including Veterans, military families, healthcare providers, and community organizations. ▪ Strong written and verbal communication skills, with the ability to tailor information to different audiences. ▪ Social media savvy (including but not limited to LinkedIn, Facebook, Instagram, and TikTok) ▪ Ability to identify obstacles to care, develop practical solutions, and lead initiatives without direct supervisory authority. ▪ Experience building partnerships, conducting community outreach, and promoting suicide prevention resources and services. This is not a remote position. The West Central AL AHEC main office is located in Eutaw, AL.(Greene County). Pay:
$38,500.00 - $42,500.00 per year
