Community Outreach Coordinator Pathway Family Services - 3.3 Visalia, CA Job Details Full-time From $26 an hour 1 day ago Benefits Health insurance Dental insurance Paid time off Vision insurance 401(k) matching Life insurance Retirement plan Qualifications Spanish Bachelor's degree Social work care coordination Community outreach case management method Full Job Description Job Summary We are seeking a dynamic and passionate Community Outreach Coordinator for Pathway; As the Community Outreach Coordinator, you are the initial contact with a community of people whose Managed Care Providers have referred them to be in one of our service programs at Pathway. The overall goal of CalAIM is to provide comprehensive care and achieve better health outcomes for the highest-need beneficiaries in Medi-Cal. The Community Outreach Coordinator (COC) plays an integral part in providing them the necessary resources, options, and coordination for a healthy care outcome. Requirements Bilingual in Spanish. Able to work with a dynamic team of Lead Care Managers who provide care coordination/case management for MCP members. Must love working with people, have a respectful and objective attitude, be friendly and outgoing. Must have a BA/BS in the social services field or a minimum of 2 years' experience. Must pass
DOJ/FBI/CAIC
clearances and health screening. Responsibilities Coordinates outreach of ECM members who are referred to Pathway Family Services. Engage eligible ECM members though progressive outreach and engagement services as described by the
DHCS ECM
Policy guide. Document all outreach attempts through MCP tracking reports and Electronic Health Record (if needed) under ECM Director supervision. Call members to facilitate visit with LCMs. Supports and promotes open communication and cooperation among participants and LCM's. Maintains a positive and encouraging relationship with the program participant to enhance their quality of life. Uses motivational interviewing, trauma-informed care, and whole person-centered care approaches. Ensures that Pathway confidentiality policies and procedures is adhered to and always followed. Attends trainings and regular meetings with CalAIM ECM Team. Attends all Pathway team meetings. Anything else requested by Cal AIM LCM Supervisors or ECM Director.
