Position Summary The Care Coordinator Assistant supports the homecare marketing and intake teams through referral coordination, outreach efforts, administrative support, and timely communication with referral sources and prospective clients. This role helps maintain relationships with healthcare partners, monitors pending referrals and case activity, and ensures prompt follow-up and accurate reporting to support patient admissions and organizational growth. Key Responsibilities
- Support marketing and intake teams with referral coordination and follow-up activities.
- Build and maintain relationships with physicians, hospitals, nursing homes, and other referral sources.
- Respond promptly to referral inquiries, phone calls, and emails from clients and referral partners.
- Monitor pending referrals and ensure cases are assigned and processed accurately in McKesson.
- Communicate patient discharge updates and scheduling concerns to marketing and intake leadership.
- Assist with community outreach events, health fairs, and preparation of promotional materials.
- Manage electronic referrals and ensure timely responses within required timeframes.
- Contact patients and referral partners for additional information, paperwork, or follow-up needs.
- Provide regular reports and updates to assigned marketing representatives and leadership.
- Support organizational growth by helping improve referral development and customer service efforts. Requirements Qualifications
- High school diploma or equivalent required; healthcare, marketing, or customer service experience preferred.
- Strong communication, organizational, and multitasking skills required.
- Proficiency in Microsoft Office as well as healthcare systems.
- Ability to maintain professionalism, confidentiality, and strong relationships with referral partners and team members.
