We are seeking a highly motivated Community Outreach Specialist to join our team. This dynamic, community-facing role bridges the gap between grassroots outreach and essential healthcare access, supporting prospective and existing members through every step of their care journey. From cold call outreach, to converting leads into active appointments, to arranging reliable transit, managing insurance alignments, and anchoring local community wellness events, you will serve as the frontline face of our organization's growth strategy. Required Qualifications

Education:

High school diploma or GED equivalent is required; a Bachelor's degree is a plus.

System Constraints Knowledge:

On-site capability at St. Bernard Hospital is required. Must understand that core internal databases (such as Meditech and Citrix) are highly secure and exclusively accessible on hospital premises.

Local Mobility:

Must possess reliable personal transportation and the absolute willingness to travel extensively throughout the Greater South Side of Chicago to drive event presence and member engagement.

Schedule Agility:

Flexibility to support extended hours, weekends, and evenings during key operational periods and marketing activations (like the Medicare AEP window).

Tech Literacy:

roficiency across standard office tools like Microsoft Office Suite (Excel, Outlook, Word, Teams).

Physical Readiness:

Capability to stand or walk for extended windows during outdoor community events, and lift/carry up to 50 lbs. for event setup and breakdown. Preferred Experience

• 3+ years of success in a sales, community outreach, patient navigation, or healthcare customer service environment. Prior exposure to Medicare, Medicaid, or Managed Care Organization (MCO) workflows. Direct experience navigating CRM platforms (Salesforce preferred) or transportation dispatch networks. High comfort level connecting with diverse populations utilizing deep cultural humility and persuasive interpersonal communication. Position has discresionary bonus (paid monthly) where up to 20% of the annual salary can be earned over the course of a year. BachelorsResponsibilitiesDrive outreach across target channels (phone, digital, referrals, community events) to turn prospects into scheduled appointments and active members. Meet or exceed established acquisition targets. Medicare Annual Enrollment Period (AEP)

Support:

Conduct targeted, proactive outreach during peak enrollment windows to guide Medicare-eligible individuals toward available wellness evaluations, preventative health goals, and enrollment services.

Transportation & Logistics Coordination:

Act with a member-first approach to book transport through Managed Care Organizations (MCOs). Track ride statuses using dedicated tools (such as Kaizen Health) and instantly deploy backup solutions for failed or delayed rides to protect continuity of care. Insurance & Primary Care Physician (PCP)

Alignment:

Coordinate directly with insurance carriers and internal departments to process and verify critical PCP switch requests. Actively resolve insurance barriers that prevent scheduling or care engagement.

Salesforce & Pipeline Management:

Maintain flawless, real-time records of all interactions, conversions, and operational tasks in Salesforce. Review data pipeline metrics to continually optimize outreach performance.

Community Advocacy & Event Execution:

Represent the organization as a trusted ambassador at neighborhood health fairs, partner activations, and local tabling events across the Greater South Side of Chicago.
