Prevention Coordinator

POSITION OVERVIEW

The Prevention Coordinator supports the implementation and delivery of prevention-focused programming at Corazón Community Services. This role is responsible for facilitating workshops, conducting outreach, supporting testing and harm reduction services, and connecting participants to critical health and social resources. Working under the direction of the Prevention Manager, the Coordinator plays a key role in executing comprehensive prevention initiatives, including sexual health education, HIV/STI testing, overdose prevention, mental health referrals, and community wellness programming. This position works directly with youth participants and collaborates across departments to ensure services are integrated, accessible, and impactful.

KEY RESPONSIBILITIES

Prevention Programming & Workshop Facilitation Facilitate workshops on sexual health, trauma-informed care, mental wellness, substance use prevention, and life skills Support implementation of Teen Pregnancy Prevention (TPP) and other evidence-based curricula Assist in developing and adapting workshop content to meet participant and community needs Deliver programming in schools, community spaces, and in-house settings HIV/STI Testing & Sexual Health Services Conduct rapid HIV/STI testing and provide pre- and post-test counseling Support outreach and education related to sexual health and risk reduction Maintain accurate, confidential testing records in compliance with state and federal guidelines Coordinate and support testing events and partner site visits Harm Reduction & Prevention Initiatives Support Naloxone distribution and overdose prevention education Assist in coordination and facilitating First Aid and CPR trainings Support implementation of the Blood Pressure Hub, including screenings and participant education Provide education on harm reduction strategies and wellness practices Participant Support & Referral Services Provide one-on-one support to participants seeking health, mental health, or social services Connect participants to mental health referral systems and external providers Assist in identifying and addressing barriers to care and participation Follow up with participants to ensure successful referrals and engagement Community Impact & Outreach Conduct outreach through workshops, school visits, community events, and tabling opportunities Represent Corazón at health fairs, resource events, and community initiatives Support recruitment of participants through schools, partners, and community engagement Assist in coordinating tattoo removal services, including participant communication and scheduling Cross-Department Collaboration & Training Collaborate with staff across departments to integrate prevention services Participate in cross-department trainings and staff meetings Support implementation of organization-wide initiatives and shared programming Maintain awareness of internal services to ensure effective referrals and coordination Program Operations & Data Tracking Maintain accurate attendance, service logs, and case documentation Track program outcomes, services delivered, and participant engagement Assist in preparing reports for internal use and funders Ensure all data is handled in compliance with confidentiality and HIPAA standards Events & Program Support Assist with planning and execution of prevention-related events, workshops, and campaigns Support logistics for trainings, outreach events, and community initiatives

Pay:

$40,000.00 - $44,000.00 per year
