Job Summary The Community Connections Coordinator will promote program services through proactive community engagement, coordinating family centered events, and conducting outreach activities with a focus on newborn encounters. This position helps increase program visibility, strengthen community relationships and provides increased accessibility in the community. The Community Connections Coordinator will be responsible for overseeing Baby TALK Times, managing Warmline calls, completing Newborn Encounters and ensuring timely follow-up for all referrals. This individual plays a vital role in introducing families to Baby TALK, building relationships, and connecting them to needed supports and services Essential Functions Conduct Newborn Encounters during the week and on Saturdays for families in the service area using the Baby TALK model and Newborn Behavioral Observations. Coordinate and participate in community outreach events, family focused gatherings and family engagement activities to promote program awareness including facilitating Baby TALK times, a weekly parent child group. Establish and maintain relationships with hospitals, clinics and community partners to identify and connect with newborn families Introduce families to baby TALK's mission providing informational materials and education about available services and program benefits Maintain and track accurate records of outreach contacts, referrals, event participation, and follow-up efforts in alignment with data tracking requirements. Collect and submit parent information and documentation forms for follow-up and referral Monitor and evaluate outreach strategies for effectiveness and adjust efforts as needed to reach program goals Represent the organization in a professional manner during all assigned work functions and events. Collaborate with other Baby TALK staff and community partners to ensure consistency and quality across locations to ensure families receive appropriate support. Answer and respond to Warmline calls in a timely and compassionate manner Meet with the Director of Family Engagement twice per month for reflective Supervision Perform other work related or assigned duties Requirements Education and Job Requirements BA in Human Services, Social Work, Early Childhood Education, public health or related field. Two years combined experience in community outreach, family services or early childhood programs preferred. Ability to interact positively with program participants, agency representatives, and the community-at-large. Other Job Requirements The ability to work flexible hours including some weekends, evenings and weekend events. Effective written and verbal communication skills, decision-making skills, time and stress management skills, organizational skills, and observational skills. Demonstrated knowledge New Org Data Base Management Systems, ISBE Prevention Initiative, and other grant systems or a willingness to learn. Ability to work cooperatively with supervisors, peers, and subordinates. Possess current First Aid and CPR cards with infant/child CPR or a willingness to complete. Ability to travel occasionally. Ability to pass agency sponsored physical examination. This position is an authorized driver and must possess own vehicle, current driver's license, good driving record, and auto insurance coverage.

THIS IS A MANDATED REPORTER POSITION.

Must have ability to operate computers and possess basic knowledge of MS Windows, Microsoft Word and other software. Reflective supervision is provided by the Director of Family Engagement regularly. The Community Connections Coordinator is evaluated based on the responsibilities described in this position description.
