The Partnership & Community Outreach Coordinator serves as the relationship-building engine of the Outreach Department and is responsible for developing, strengthening, and maintaining relationships with community organizations, healthcare providers, schools, social service agencies, and other referral partners.

RESPONSIBILITIES

Identify potential referral sources. Develop strategies for engaging both high-potential and previously inactive referral sources. Identify gaps in the current referral network and opportunities for expansion Conduct professional outreach calls, emails, and other approved communications to prospective referral sources. Communicate the organization's value proposition clearly and professionally. Track outreach attempts, responses, meetings, and outcomes. Follow up consistently with prospective partners to move relationships toward active referral partnerships. Schedule and participate in meetings with prospective and existing community partners. Document outcomes of meetings and outreach efforts.
