Job Description

Community Outreach Coordinator The Community Outreach Coordinator will function as part of the multidisciplinary team and will provide services to patients of all ages. Outreach is provided to ambulatory patients, as well as community members. The Community Outreach Coordinator will coordinate appointment schedules between the Population Health Team, Primary Care Providers and GVMH patients; utilize Microsoft applications to interact with Payer data; document Population Health Team activity in the electronic medical record, maintain Payer data spreadsheets, and support value-based care initiatives and population health strategies organization wide.
Duties:
Completes outreach to GVMH patients due to a well visit or preventive screening and schedules appointments as appropriate Ensure Payer data spreadsheets are well-maintained, updates Payer Portals documents, and Population Health Team activity in the electronic medical record Collaborate with Provider Offices on a daily basis to monitor progress . Collaborates with Provider Offices and Centralized Scheduling Staff to ensure Provider preferences are maintained and accurate Demonstrates knowledge of cultural diversity and/or disabilities for GVMH patients and displays s ensitivity to customers Complete administrative tasks to ensure proper and organized record keeping is completed
Requirements:
High School diploma or equivalent 1-3 Years of experience preferred Why Join Us?
